摘要
目的:分析甲状腺次全切除术后应用柴胡疏肝散合二陈汤对甲状腺癌患者的疗效。方法:选取2018年1月—2020年1月本院行甲状腺次全切除术的甲状腺癌患者60例,按随机数字表法分为中医组(n=30)和西医组(n=30)。西医组予以左甲状腺素钠片口服,50μg/次,1次/d;中医组在西医组的基础上予以柴胡疏肝散合二陈汤治疗,1剂/d,分早晚两次服。对比两组并发症发生率、复发率及治疗前后中医证候积分、血清促甲状腺激素(TSH)和甘油三酯(TG)水平。结果:两组治疗前自汗和口燥咽干评分比较无显著差异(P>0.05),治疗后,两组自汗和口燥咽干评分均有所减低,且中医组较西医组低(P<0.05);两组治疗前血清TSH和TG水平比较无显著差异(P>0.05),治疗后,两组血清TSH和TG水平均有所降低,且中医组较西医组低(P<0.05);中医组并发症发生率为3.33%,较西医组的26.67%低(P<0.05);中医组复发率为3.85%,西医组为25.00%,两组比较无显著差异(P>0.05)。结论:柴胡疏肝散合二陈汤应用于甲状腺癌患者行甲状腺次全切除术后,可改善临床症状,降低血清TSH和TG水平,降低并发症发生率,预防复发,值得临床应用。
Objective:To analyze the effect of Chaihu Shugan powder combined with Erchen decoction on patients with thyroid cancer after subtotal thyroidectomy.Methods:A total of 60 patients with thyroid cancer who underwent subtotal thyroidectomy in our hospital from January 2018 to January 2020 were selected and randomly divided into the traditional Chinese medicine group(n=30)and the western medicine group(n=30).The western medicine group was treated with levothyroxine sodium tablets,50μg each time,once a day.The traditional Chinese medicine group was treated with Chaihu Shugan powder and Erchen decoction on the basis of the western medicine group(1 dose/day,twice in the morning and evening).The incidence of complications,recurrence rate,TCM syndrome score,serum thyroid-stimulating hormone(TSH),and triglyceride(TG)levels before and after treatment were compared between the two groups.Results:There was no significant difference in the scores of spontaneous sweating,dry mouth,and dry throat between the two groups before treatment(P>0.05).Compared with before treatment,the scores of spontaneous sweating,dryness of mouth and throat in the two groups were decreased,and the scores of the TCM group were lower than that of the western group(P<0.05).There were no significant differences in serum TSH and TG levels between the two groups before treatment(P>0.05).After treatment,serum TSH and TG levels in the two groups were decreased,and the TCM group was lower than the western group(P<0.05).The complication rate of the TCM group was 3.33%,which was lower than that of the western medicine group(26.67%,P<0.05).The recurrence rates were 3.85%in the TCM group and 25.00%in the western medicine group,respectively.Conclusion:The application of Chaihu Shugan powder combined with Erchen decoction in thyroid cancer patients after subtotal thyroidectomy can improve clinical symptoms,reduce serum TSG and TG levels,reduce the incidence of complications,and prevent a recurrence,which is worth clinical application.
