摘要
目的分析评价启动城市公立医院综合改革前后公立医院的效果,探究运行效率影响因素,科学合理配置医疗资源,提升医院运行效率。方法根据广州市南沙区11家公立医院运行数据,描述性方法纵向分析2015—2019年广州市南沙区公立医院总体运行情况,选取投入产出指标构建数据包络分析评价模型,分析医院运行效率及变动情况。结果公立医院综合改革以来,运行总体良好,效益持续提升,收入结构渐趋合理。横向分析改革前2015和改革后2019年公立医院运行效率,2015年和2019年的综合有效、技术有效分别有8家医院(73%)、9家医院(82%),技术无效的医院存在不同程度投入不足问题。2015年和2019年分别有8家医院(73%)、10家医院(82%)规模有效,规模无效的医院均为规模递增。结论改革成效初显,运行效率有所提升,但存在投入规模不足,制约医院业务发展,应进一步加强区域卫生资源规划,提升资源配置效率。
Objective This article endeavors to analyze the differences before and after implementation of comprehensive reform in public hospitals,and to explore the factors associated with hospital operation efficiency.The results will be used to allocate healthcare resources scientifically and rationally and to improve hospital operation efficiency.Methods On the basis of the operation data from 11 public hospitals in Guangzhou Nansha District,descriptive analysis was applied to longitudinally describe the general performances of public hospitals in Guangzhou Nansha District in 2015 to 2019.The input and output indexes were selected to construct data development analysis models for analysis of hospital operation efficiency and variations.Results Since the implementation of comprehensive reform,the public hospitals have been generally well-functioning,the efficiency has been improved and the income structure has been optimized.Horizontal analysis of the public hospitals operation efficiency before and after the reform found that eight(73%)and nine(82%)hospitals were efficient generally and technically in 2015 and 2019 respectively.Those hospitals found to be inefficient technically suffer from insufficient investment to various extent.In 2015 and 2019,eight(73%)and ten(82%)hospitals were found with scale efficiency respectively.Those without scale efficiency were regarded as increasing returns to scale.Conclusion The reform has achieved initial success and the operation efficiency has been improved.However,the problem of insufficient investment restricted the hospital development.Regional health care resources planning should be consolidated to improve the efficiency of resource allocation.
