非增强和增强核磁共振定位股前外侧皮瓣穿支的临床对照研究 被引量:9

Non-enhanced and enhanced magnetic resonance angiography in perforator location of anterolateral thigh flap:a clinical comparative study

摘要 目的探讨非增强核磁共振(non-enhanced magnetic resonance angiography,NE-MRA)在股前外侧穿支皮瓣穿支定位中的临床应用价值。方法自2018年5月至2019年10月,我们对19例拟行股前外侧皮瓣移植术的患者,术前均进行NE-MRA和增强核磁共振(contrast-enhanced magnetic resonance angiography,CE-MRA)穿支定位,分别与术中定位进行对比,记录穿支数量、走行、来源,并采用Likert量表评估两种影像学检查的图像质量。结果NE-MRA组的术前定位敏感度为86.7%(39/45),阳性预测值为92.9%(39/42);CE-MRA组的术前定位敏感度为88.9%(40/45),阳性预测值为93.0%(40/43),差异均无统计学意义(P>0.05)。两组穿支走行和来源结果均与术中结果相符。在图像质量方面,NE-MRA组Likert评分为2.3±0.3,低于CE-MRA组3.5±0.4,差异有统计学意义(P<0.05)。结论NE-MRA应用在股前外侧皮瓣移植术中具有穿支定位敏感度高、无创、安全性高的优点,便于在基层医院推广应用。 Objective To explore the clinical application value of non-enhanced magnetic resonance angiography(NE-MRA)in anterolateral thigh flap perforator location.Methods From May 2018 to October 2019,we performed NE-MRA and contrast-enhanced magnetic resonance angiography(CE-MRA)for perforator localization before anterolateral thigh flap transplantation in 19 patients,and compared the examination results with intraoperative localization.The number,course and source of perforators were recorded,and the image quality of the two imaging examinations was evaluated by Likert scale.Results The preoperative localization sensitivity of NE-MRA group was 86.7%(39/45),and the positive predictive value was 92.9%(39/42);the preoperative localization sensitivity of CE-MRA group was 88.9%(40/45),and the positive predictive value was 93.0%(40/43).The differences were not statistically significant(P>0.05).The course and source of perforator in both groups were consistent with the intraoperative results.In terms of image quality,Likert score of NE-MRA group was 2.3±0.3,which was lower than that of CE-MRA group 3.5±0.4.The difference was statistically significant(P<0.05).Conclusion NE-MRA has the advantages of high sensitivity of perforator localization,non-invasive and high safety in the application of anterolateral thigh flap transplantation,which is easy to be popularized and applied in primary hospitals.
