血小板体积、血小板分布宽度及红细胞分布宽度与早发冠心病的关系 被引量:7

Relationship between meanplatelet volume,platelet distribution width,red blood cell distribution width and premature coronary heart disease

摘要 目的分析血小板体积(MPV)、血小板分布宽度(PDW)、红细胞分布宽度(RDW)与早发冠心病(PCAD)的关系。方法选取2018年3月—2019年3月于郑州人民医院收治的98例冠心病患者,按照发病年龄分为PCAD组(男性<55岁、女性<65岁)与对照组(男性≥55岁,女性≥65岁),PCAD组中共39例,对照组中共49例,两组均进行血细胞分析,观察两组基本病理资料、MPV、PDW及RDW等与PCAD的关系。结果两组患者性别、身体质量指数(BMI)、冠心病家族史、低密度脂蛋白胆固醇(LDL-C)对比差异无统计学意义(P>0.05);而两组患者年龄、吸烟史、高血压病史、高密度脂蛋白胆固醇(HDL-C)、尿酸、同型半胱氨酸(Hcy)、三酰甘油(TG)对比差异有统计学意义(P<0.05);PCAD组MPV、PDW、红细胞体积大小的变异系数(RDW-CV)、红细胞分布宽度的标准差(RDW-SD)水平明显高于对照组(P<0.05);Logistic多因素回归分析显示,MPV、PDW、RDW是PCAD发病的独立危险因素(P<0.05)。结论 MPV、PDW、RDW作为PCAD发病的独立危险因素,具有一定的诊断价值。 Objective To analyze the relationship between mean platelet volume(MPV), platelet distribution width(PDW), red blood cell distribution width(RDW) and premature coronary heart disease(PCAD).Methods Between March 2018 and March 2019, 98 patients with coronary heart diseasewere recruited and divided into PCAD group(male <55 years old, female <65 years old, 39 cases) and control group(male ≥55 years old, female ≥65 years old, 49 cases) according to the age of onset. Both groups were subjected to blood cell analysis, and the relationship between the basic pathological data, MPV, PDW, RDW and PCAD was observed.Results There were no significant differences in gender, body mass index(BMI), family history of coronary heart disease, and low-density lipoprotein cholesterol(LDL-C) between the two groups(P>0.05), while there were significant differences in age, smoking history, history of hypertension, high-density lipoprotein cholesterol(HDL-C), uric acid, homocysteine(Hcy), and triacylglycerol(TG)(P<0.05). PCAD group had significantly higher MPV, PDW, coefficient of variation of RDW(RDW-CV) and standard deviation of RDW(RDW-SD)thanthe control group(P<0.05). Multivariate Logistic regression analysis showed that MPV, PDW, and RDW are independent risk factors for PCAD(P<0.05).Conclusion MPV, PDW and RDW are independent risk factors for PCAD, suggesting that the three have certain diagnostic value.
