摘要
目的应用舒肝解郁胶囊治疗阿尔茨海默病(AD)伴抑郁患者,观察其临床疗效,检测血液5-羟色胺(5-HT)、多巴胺(DA)水平,分析对氧化应激的影响。方法选择2017年5月—2018年7月期间收治的120例AD伴抑郁患者,随机分为对照组和治疗组,每组各60例,对照组给予艾司西酞普兰,治疗组在此基础上同时应用舒肝解郁胶囊进行治疗。评估患者临床疗效,应用治疗副反应量表(TESS)、康奈尔痴呆抑郁量表(CSDD)、汉密尔顿抑郁量表(HAMD)、简易职能精神状态量表(MMSE)和日常生活能力评估量表(ADL)分别评估患者不良反应、认识功能、抑郁状况及日常生活能力,检测血清5-HT、DA、超氧化物歧化酶(SOD)、丙二醛(MDA)水平。结果治疗后治疗组患者总有效率81.67%高于对照组68.33%,两组比较差异有统计学意义(P<0.05)。两组患者TESS评分比较,差异无统计学意义(P>0.05)。两组患者治疗后MMSE、CSDD、HAMD和ADL评分结果均优于治疗前,差异有统计学意义(P<0.05);且治疗组治疗后MMSE、SDS、HAMD和ADL评分均优于对照组,差异有统计学意义(P<0.05)。两组患者治疗后血清5-HT、DA和SOD均高于治疗前,血清MDA均低于治疗前,两组治疗前后组内比较,差异有统计学意义(P<0.05);且治疗组治疗后血清5-HT、DA和SOD均高于对照组,血清MDA低于对照组,两组比较差异有统计学意义(P<0.05)。结论舒肝解郁胶囊治疗AD伴发抑郁患者,临床疗效显著提高,抑郁状况显著改善,且上调5-HT、DA和SOD水平,降低MDA含量,并具有较高的安全性。
Objective To evaluate the effect of Shugan Jieyu Capsules on improving the cognition and emotion in Alzheimer’s disease(AD)patients with depression and its effect on oxidative stress by evaluating serum levels of 5-HT and DA.Methods A total of 120 AD patients with depression treated in the hospital from May,2017 to July,2018 were included and randomly divided into the control group and the research group(60 patients per group).The control group was given escitalopram,and the research group was given escitalopram combined with Shugan Jieyu Capsules.The clinical efficacy was evaluated.The Treatment Emergent Symptom Scale(TESS),Cornell Scale for Depression in Dementia(CSDD),Hamilton Depression Scale(HAMD),Mini-Mental State Examination(MMSE)and Activities of Daily Living(ADL)were used to evaluate the adverse events,cognitive function,depression and daily living ability,and detected 5-hydroxytryptamine(5-HT),dopamine(DA),superoxide dismutase(SOD)and malondialdehyde(MDA)levels.Results The total effective rate was 81.67%in the research group,which was higher than that(68.33%)in the control group,with statistical significance(P<0.05).There was no significant difference in the TESS score between the two groups(P>0.05).The scores of MMSE,CSDD,HAMD and ADL were improved after treatment,with statistical significance(P<0.05);and the research group had higher scores than the control group,with statistical significance(P<0.05).After treatment,the levels of serum 5-HT,DA and SOD increased and serum MDA decreased,and there were significant differences before and after treatment in both groups(P<0.05);and the research group had higher levels of serum 5-HT,DA and SOD and lower level of MDA than the control group,with statistical significance(P<0.05).Conclusion Shugan Jieyu Capsules can significantly improve depression,up-regulate the levels of 5-HT and DA,and reduce MDA,with a good and safe effect.
