摘要
目的探讨卒中神经源性膀胱导尿的老年患者尿路感染的病原学特点及危险因素。方法选取218例神经内科住院的卒中神经源性膀胱导尿老年患者为研究对象,对导致患者尿路感染发生的相关因素进行统计学分析。结果218例卒中神经源性膀胱导尿的老年患者中,发生尿路感染的共有67例,发生率为30.73%。感染患者的尿液分离出致病菌89株,其中革兰阳性菌有17株,占19.10%;革兰阴性菌有51株,占57.30%;真菌有21株,占23.60%。多因素Logistic回归分析表明,女性、年龄≥75岁、合并糖尿病、康复介入时间≥7 d、留置尿管时间≥7 d及卒中次数≥2次是导致卒中神经源性膀胱导尿的老年患者尿路感染发生的危险因素(OR>1,P<0.05)。结论卒中神经源性膀胱导尿老年患者尿路感染的主要致病菌为革兰阴性菌及真菌,其与患者的性别、年龄、并发症、康复介入时间、留置尿管时间及卒中的病情等多种因素有关,应引起我们的重视。
Objective To explore the etiological characteristics and risk factors of urinary tract infection in elderly patients with neurogenic bladder catheterization after stroke.Methods A total of 218 elderly patients with neurogenic bladder catheterization after stroke who were hospitalized in neurology department were selected as the research objects,and the related factors leading to the occurrence of urinary tract infections were statistically analyzed.Results Among 218 elderly patients with neurogenic bladder catheterization,67 cases had urinary tract infection,and the incidence was 30.73%.89 strains of pathogenic bacteria were isolated from the urine of infected patients,of which 17 strains(19.10%)were Gram-positive bacteria,51 strains(57.30%)were Gram-negative bacteria and 21 strains(23.60%)were fungi.Multivariate Logistic regression analysis showed that female,age≥75 years old,diabetes mellitus,rehabilitation intervention time≥7 days,indwelling catheter time≥7 days and stroke frequency≥2 times were risk factors for urinary tract infection in elderly patients with neurogenic bladder catheterization after stroke(OR>1,P<0.05).Conclusion Gram-negative bacteria and fungi are the main pathogens of urinary tract infection in elderly patients with neurogenic bladder catheterization after stroke,which are related to many factors such as patient’s gender,age,complications,rehabilitation intervention time,indwelling catheter time,the condition of stroke and so on.This should arouse our attention.
