摘要
目的回顾性分析不同年龄组三叉神经带状疱疹后神经痛患者的临床特征。方法收集2013年1月至2020年7月,在中国医科大学航空总医院疼痛中心住院的三叉神经带状疱疹后神经痛患者118例,年龄、性别不限,以65岁为界,分为老年组和非老年组,用视觉模拟评分法(VAS)记录患者入院时的疼痛程度;使用非接触式红外测温仪评估面部皮肤温度;用大头针或vonFrey纤维评估患者的感觉异常或痛觉异常,并且对患者的年龄、性别、合并症、眼部并发症及疼痛部位进行收集和统计学分析。结果最终纳入分析的患者共111例,两组患者入院时VAS评分及病变部位差异均无统计学意义(P均>0.05);老年组痛觉过敏发生率高于非老年组,差异有统计学意义(26.2%比12.0%,P<0.05),但两组患者疼痛性质及痛觉超敏发生率差异均无统计学意义(P均>0.05)。结论老年三叉神经带状疱疹后神经痛患者痛觉过敏发生率较高。
Objective The clinical characteristics of trigeminal postherpetic neuralgia in different age groups were retrospectively analysed.Methods A collection of 118 patients with trigeminal postherpetic neuralgia who were hospitalized in the Pain Center of Aviation General Hospital of China Medical University from January 2013 to July 2020,regardless of age and gender,were divided into elderly group(≥65 years)and non-elderly group(<65 years).Visual analogue scale(VAS)was recorded at admission.Skin temperature was assessed by non-contact infrared thermometers.Patient's paresthesia or hyperalgesia and allodynia were evaluated by Pins or vonFrey fibers,and patient's age,gender,comorbidities,ocular complications and distribution of pain were collected and analyzed.Results A total of 111 cases were included in the final analysis.There were no differences in VAS and diseased regions at admission in the two groups(all P>0.05).Compared with the non-elderly group,the incidence of hyperalgesia was significantly higher in the elderly group(26.2%vs.12.0%,P<0.05).No difference was found in pain quality and the incidence of pain hypersensitivity between the two groups(all P>0.05).Conclusion Elderly patients with trigeminal postherpetic neuralgia has higher incidence of hyperalgesia.
