摘要
目的:评价某三级甲等医院(以下简称“该院”)门诊及急诊头孢克肟的用药合理性。方法:以头孢克肟口服剂型的药品说明书为基础,参考《抗菌药物临床应用指导原则(2015年版)》《医院处方点评管理规范(试行)》及相关文献等,建立头孢克肟合理使用评价指标、评价内容及评价标准。采用加权优劣解距离法(technique for order preference by similarity to an ideal solution,TOPSIS)对2019年10—11月该院门诊及急诊507张使用头孢克肟处方中头孢克肟的用药合理性进行评价。结果:507张使用头孢克肟的处方中,与最优方案接近程度(Ci)为1的处方共127张(占25.0%),Ci在0.8~<0.9范围内的处方共5张(占1.0%),Ci在0.7~<0.8范围内的处方共267张(占52.7%),Ci在0.6~<0.7范围内的处方共96张(占18.9%),Ci在0.5~<0.6范围内的处方共12张(占2.4%)。结论:该院门诊及急诊头孢克肟的使用较为合理,但尚需在处方医师权限、适应证方面进一步规范头孢克肟的临床应用。
OBJECTIVE:To evaluate the application rationality of cefixime in outpatient and emergency department of a gradeⅢ,level A hospital(hereinafter referred to as“the hospital”).METHODS:According to cefixime instructions and Guidelines for Clinical Application of Antibiotics(2015 Edition),Management Standards for Hospital Prescription Review(Trial)and related literature,the evaluation indicators,evaluation content and evaluation criteria for the rational application of cefixime were established.Application rationality of cefoxime in 507 prescriptions with cefixime in outpatient and emergency department of the hospital from Oct.to Nov.2019 was evaluated by weighted preference by similarity to an Ideal solution(TOPSIS).RESULTS:Of the 507 prescriptions with cefixime,there were 127 prescriptions(25.0%)with a degree of closeness(Ci)of 1 to the optimal plan,there were 5 prescriptions(1.0%)within the Ci range of 0.8-<0.9,267 prescriptions(52.7%)within the Ci range of 0.7-<0.8,96 prescriptions(18.9%)within the Ci range of 0.6-<0.7,and 12 prescriptions(2.4%)within the Ci range of 0.5-<0.6.CONCLUSIONS:The application of cefixime in the outpatient and emergency departments of this hospital is rational,but the rational application of cefixime in the authority of the prescribing clinicians and indications is still needed.
