摘要
目的:了解北京市27家三级中医医院抗菌药物使用的现状。方法:抽取2018年北京市27家三级中医医院临床抗菌药物使用数据,对门诊、急诊和住院患者抗菌药物的使用率,住院患者抗菌药物使用强度,Ⅰ类切口手术围手术期抗菌药物预防性使用情况以及病原微生物送检率等相关数据进行统计分析。结果:2018年北京市27家三级中医医院门诊、急诊和住院患者抗菌药物使用率分别为6.13%、26.16%和43.19%,住院患者抗菌药物使用强度为33.34 DDDs/(100人•d);Ⅰ类切口手术围手术期抗菌药物预防性使用率为23.65%,预防性使用抗菌药物在术前0.5~2 h给药者所占比例为89.10%,抗菌药物预防性使用疗程≤24 h者所占比例为42.14%;接受抗菌药物治疗的住院患者病原微生物送检率为58.92%,使用限制使用级抗菌药物治疗的住院患者微生物送检率为61.58%,使用特殊使用级抗菌药物治疗的住院患者微生物送检率为79.74%。结论:北京市27家三级中医医院抗菌药物的使用相对合理,但仍需对个别抗菌药物指标(如抗菌药物使用强度)加强管理,促进临床抗菌药物的合理应用。
OBJECTIVE:To investigate the application of antibiotics in 27 three-level hospitals of traditional Chinese Medicine in Beijing.METHODS:According to the application data of antibiotics reported by 27 three-level hospitals of traditional Chinese Medicine in Beijing in 2018,statistical analysis was conducted on application rate of antibiotics in outpatient,emergency and inpatient department,drug use intensity in inpatients,prophylactic application of prophylactic antibiotics in typeⅠincision surgery during perioperative period and detection rate of pathogenic microorganism.RESULTS:The application rate of antibiotics in outpatient,emergency and inpatient department in 27 three-level hospitals of traditional Chinese Medicine in Beijing was respectively 6.13%,26.16%and 43.19%,the drug use intensity in inpatients was 33.34 DDDs/(100 persons•d).The application rate of prophylactic antibiotics in typeⅠincision surgery during perioperative period was 23.65%,the average proportion of prophylactic use of antibiotics 0.5-2 h before surgery was 89.10%,the proportion of patients with prophylactic antibiotics treatment course≤24 h was 42.14%.The detection rate of pathogenic microorganism in inpatients receiving antibiotics was 58.92%,that of inpatients treated with restricted-use antibiotics was 61.58%,and that of inpatients treated with special-use antibiotics was 79.74%.CONCLUSIONS:The application of antibiotics in 27 three-level hospitals of traditional Chinese Medicine in Beijing is basically rational,but it is still necessary to strengthen the management of individual indicators of antibiotics(such as drug use intensity)to promote the rational use of clinical antibiotics.
