创伤性骨关节炎的炎症机制研究进展 被引量:8

Research progress in inflammatory mechanism of post-traumatic osteoarthritis

摘要 创伤性骨关节炎(PTOA)是由于急性关节损伤引起肿胀、积液、疼痛、血肿等症状的疾病。尽管研究显示多种分子和生物学机制参与了PTOA的发病机制和疾病进展,但关节损伤后急性炎症反应的激活被认为在PTOA的发生、发展过程中起着关键作用。对PTOA患者和动物模型研究在一定程度上揭示了关节损伤后炎症介质产生、炎症通路激活、炎症细胞浸润的过程和炎症反应对PTOA发生、发展的影响,这对进一步研制预防、治疗PTOA的措施与药物具有重要意义。笔者从体内与体外研究模型、局部炎症表现、炎症介质、全身炎症影响因素、炎症信号通路等方面综述PTOA的炎症机制研究进展,为临床防治PTOA提供参考。 Post-traumatic osteoarthritis(PTOA)is a condition with symptoms of swelling of damaged joints,joint effusion,pain,and even joint hematoma caused by acute joint injury.Although studies have shown that a variety of molecular and biological mechanisms are involved in PTOA pathogenesis and disease progression,activation of acute inflammatory response after joint injury is thought to play a key role in the occurrence and development of PTOA.To some extent,the study of PTOA patients and animal models reveals the process of inflammatory mediators production,activation of inflammatory pathways,infiltration of inflammatory cells and the influence of inflammatory response on the occurrence and development of PTOA after joint injury,which is of great significance for the further development of preventive and therapeutic PTOA measures and drugs.The authors explore the progress of inflammatory mechanism of PTOA by reviewing the in vivo and in vitro research models,local inflammatory manifestations,inflammatory mediators,systemic inflammatory influencing factors,and inflammatory pathways to provide a reference for clinical prevention and treatment in PTOA.
