福多司坦治疗慢性阻塞性肺疾病合并肺纤维化效果及对肺功能、肺纤维化和血清涎液化糖链抗原-6、转化生长因子β1、趋化因子13水平的影响 被引量:15

摘要 目的探讨福多司坦治疗慢性阻塞性肺疾病(COPD)合并肺纤维化效果及对肺功能、肺纤维化和血清涎液化糖链抗原-6(KL-6)、转化生长因子β1(TGF-β1)、趋化因子13(CXCL13)水平的影响。方法我院收治的106例COPD合并肺纤维化患者,按照随机数字法分为对照组和观察组各53例,对照组给予乙酰半胱氨酸治疗,观察组给予福多司坦治疗,比较两组临床疗效、肺功能、肺纤维化指标、KL-6、TGF-β1、CXCL13水平及不良反应发生率。结果观察组治疗有效率高于对照组(P<0.05);治疗后观察组FVC、FEV1、FEV1/FVC%水平高于对照组,血清HA、LN、IL-8、ICAM-1、KL-6、TGF-β1、CXCL13水平低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论福多司坦治疗COPD合并肺纤维化的效果显著,能够改善肺功能和肺纤维化,降低血清KL-6、TGF-β1、CXCL13水平。 Objective To investigate the effect of fudosteine in the treatment of chronic obstructive pulmonary disease(COPD)complicated with pulmonary fibrosis,and its effect on pulmonary function,pulmonary fibrosis and serum levels of salivary liquefying carbohydrate antigen-6(KL-6),transforming growth factor-β1(TGF-β1)and chemokine 13(CXCL13).Methods One hundred and six COPD patients with pulmonary fibrosis admitted to our hospital were selected as study subjects.They were randomly divided into control or observation group,53 in each group.The control group was treated with acetylcysteine,and the observation group was treated with fudosteine.The clinical efficacy,lung function,pulmonary fibrosis indexes,and levels of KL-6,TGF-β1 and CXCL13 were compared between the two groups.The incidence of adverse reactions was also compared between the two groups.Results The effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the levels of FVC,FEV1 and FEV1/FVC%in the observation group were higher than those in the control group and the levels of serum HA,LN,IL-8,ICAM-1,KL-6,TGF-β1 and CXCL13 in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The therapeutic effect of fudosteine in the treatment of COPD combined with pulmonary fibrosis is significant.It can improve the pulmonary function and pulmonary fibrosis,and reduce the serum levels of KL-6,TGF-1,and CXCL13.
