摘要
目的探究中性粒细胞活化与血栓性血小板减少性紫癫(thrombotic thrombocytopenic purpura,TTP)活动性的相关性。方法收集2012年2月—2013年4月之间TTP患者26例,并以22例健康受试者作对照,获得了受试者的年龄、性别及临床指标等特征数据,以评估相关性。结果TTP疾病相关临床指标ADAMTS13酶活性、血红蛋白及血小板分别急剧下降了94.81%,40.62%和85.69%,而相关的疾病炎症反应临床指标白细胞、中性粒细胞分别显著增加了1.14倍和1.65倍。TTP组患者的PMN弹性蛋白酶浓度是健康组的12.44倍,差异具统计学意义(t=30.959,P<0.001),且治疗后的TTP患者其PMN弹性蛋白酶含量显著下降了86.65%。Spearman相关性分析发现,ADAMTS13活性、激活因子B、过敏毒素C3a均与血浆PMN弹性蛋白酶显著正相关(P<0.05)。相反,PMN弹性蛋白酶显著负相关于血红蛋白(r=-0.356,P<0.05)和血小板(r=-0.522,P<0.05)。结论血浆中中性粒细胞的活化与TTP密切相关,主要表现为PMN弹性蛋白酶与TTP活性标志物及补体的激活显著相关,而治疗TTP后可显著降低PMN弹性蛋白酶水平。
Objective To explore the correlation between neutrophil activation and acute thrombotic thrombocytopenic purpura(TTP)activity.Methods Data of 26 patients with TTP from February 2012 to April 2013 were collected and compared with 22 healthy subjects in terms of age,gender and clinical indicators to evaluate the correlation.Results In this study,TTP related clinical indicators as ADAMTS13 enzyme activity,hemoglobin value and platelet counts significantly decreased by 94.81%,40.62%and 85.69%,respectively.While related clinical indicators of inflammatory reaction as leukocytesand neutrophils significantly increased by 1.14 times and 1.65 times,respectively.The concentration of PMN elastase in the TTP group was 12.44 times higher than that in the healthy group,and the difference was statistically significant(P<0.001).The content of PMN elastase in TTP patients decreased by 86.65%after treatment.Spearman correlation analysis showed that ADAMTS13 activity,activator B and anaphylaxis toxin C3 a were positively correlated with plasma PMN elastase(P<0.05).In contrast,PMN elastase was significantly negatively correlated with hemoglobin(P<0.05)and platelets(P<0.05).Conclusion The activation of neutrophils in plasma was closely related to TTP,mainly manifested as significant correlation between PMN elastase,TTP active markers and complement activation.However,the elastase level of PMN was significantly decreased after TTP treatment.
