摘要
目的探讨超声诊断胎儿先天性无肛门症的价值及超声产科筛查中肛门正常测值情况。方法回顾性分析2016年10月~2019年7月在江西省赣州市妇幼保健院进行产前超声筛查的孕龄20周以上6000例胎儿的临床资料,分析其产前超声诊断结果,观察胎儿的超声诊断结果、肛门括约肌声像图表现、正常胎儿肛门括约肌直径、周长及面积测量值与孕龄的相关性。结果6000例胎儿中,5996例出现肛门“靶环征”,其余4例均未探及肛门“靶环征”声像,并且在引产、流产或分娩后均证实为先天性无肛门症状,其中1例为单纯闭锁,3例合并其他畸形及异常;正常胎儿肛门括约肌直径、周长及面积测量值与孕龄呈正相关(P<0.05)。结论超声诊断胎儿先天性无肛门症及产科筛查中肛门正常测值具有较高的价值,但仍然需要加强对超声诊断的改进,并提升检测人员的能力,从而提升诊断率。
Objective To investigate the value of ultrasound in diagnosis of fetal congenital imperforate anus and the normal measurement of the anus in ultrasound obstetric screening.Methods The clinical data of 6000 fetuses with a gestational age of 20 weeks or above admitted to Ganzhou Maternal and Child Health Hospital of Jiangxi Province from October 2016 to July 2019 were retrospectively analyzed.Prenatal ultrasound diagnosis was analyzed,and the ultrasound diagnosis results of the fetuses and the sonographic appearance of anal sphincter,the correlation between anal sphincter diameter,circumference,and area measurement in healthy fetuses and gestational age were observed.Results Among the 6000 fetuses,5996 of them had anal target sign,and the other 4 cases did not detect this sign,and they were all confirmed to be imperforate anus after induction,abortion or delivery,including 1 case of simple atresia,3 cases of other malformations and abnormalities.The anal sphincter diameter,circumference,and area in healthy fetuses were positively correlated with gestational age(P<0.05).Conclusion Ultrasound diagnosis has a high value in fetal congenital imperforate anus and normal measurement of anus in obstetric screening.However,it is still necessary to strengthen the improvement of ultrasound diagnosis and improve the ability of inspectors to increase the diagnosis rate.
