摘要
目的探究在非全麻下应用高强度聚焦超声联合缩宫素治疗子宫肌瘤的疗效。方法回顾性分析我院收治的100例子宫肌瘤患者(包括多发性子宫肌瘤)的临床资料。所有患者均非全麻下进行超声聚焦联合缩宫素治疗。高强度聚焦超声治疗术前及术后立即行超声造影,显示消融率,评价消融范围,并对肌瘤行彩色多普勒血流成像对比。术后3个月、6个月对患者行腹部彩色超声评价治疗后子宫肌瘤的体积及子宫肌瘤血供情况。结果56例月经过多患者,有48例经量明显减少,有效率86.0%;35例痛经患者治疗后有25例痛经明显减轻,个别消失,有效率71.4%;18例不规则阴道流血患者治疗后有13例经量接近正常,有效率72%。92例患者107个肌瘤消融治疗后立即超声造影显示子宫肌瘤体积为(24.3±5.7)cm^3,肌瘤内无血流灌注区体积(20.2±4.3)cm^3,消融率85%(67%~100%)。在治疗后的6个月子宫肌瘤体积逐渐缩小,子宫肌瘤体积缩小率逐渐增加。所有治疗患者均无严重并发症发生。结论高强度超声聚焦联合缩宫素治疗子宫肌瘤,是一种安全、有效的治疗子宫肌瘤方法。
Objective To explore the therapeutic effect of high intensity focused ultrasound combined with oxytocin on hysteromyoma under non general anesthesia.Methods The clinical data of 100 cases of hysteromyoma(including multiple hysteromyoma)in our hospital were analyzed retrospectively.All patients were treated by ultrasound focusing combined with oxytocin without general anesthesia.Contrast-enhanced ultrasound was performed before and immediately after high-intensity focused ultrasound treatment to show the ablation rate,evaluate the ablation range,and compare the color Doppler flow imaging of myoma.Three and six months after operation,the volume and blood supply of hysteromyoma were evaluated by abdominal color ultrasound.Results In 56 cases of menorrhagia,48 cases of menorrhagia decreased significantly,with an effective rate of 86.0%;After treatment,25 cases of dysmenorrhea were significantly relieved,with an effective rate of 71.4%;After treatment,13 of 18 patients with irregular vaginal bleeding had normal menstrual volume,and the effective rate was 72%.The volume of hysteromyoma was(24.3±5.7)cm^3,the volume of non perfusion area was(20.2±4.3)cm^3,and the ablation rate was 85%(67%~100%).In the 6 months after treatment,the volume of uterine leiomyoma decreased gradually,and the reduction rate of uterine leiomyoma increased gradually.No serious complications occurred in all patients.Conclusion High intensity ultrasound focusing combined with oxytocin is a safe and effective method to treat hysteromyoma.
