摘要
目的构建适合我国国情的患者胰岛素注射循证实践方案,为胰岛素注射管理提供参考。方法以JBI循证卫生保健模式及奥马哈系统为理论框架,结合医护患访谈、临床情景分析、循证资源数据库检索、专家咨询等方法,对纳入的9篇胰岛素注射相关指南进行深入评价、分析,初步构建患者胰岛素注射循证实践方案。结果糖尿病患者胰岛素注射循证实践方案主要包括4个方面:组建以糖尿病专科护士为主导的多学科团队、制订《胰岛素注射最佳实践信息手册》并进行胰岛素注射流程再造、组织临床护士进行相关培训以及实施临床干预。最佳实践信息手册又涉及胰岛素注射的指导者、治疗与程序、教育与咨询、个案管理及监测5个方面的17条推荐意见。结论该实践方案构建过程科学,适宜于临床推广应用,可为糖尿病患者胰岛素注射的自我管理提供参考依据。
Objective To construct an evidence-based practice scheme for insulin injection in patients according to the realities of China, so as to provide a reference for insulin injection management. Methods In light of the theory of JBI Model of Evidence-based Healthcare and Omaha System, combined with doctor-patient interviews, clinical scenario analysis, retrieval from evidence-based resource database, expert consultation and other methods, 9 papers of insulin injection guidelines were further evaluated and analyzed to construct a preliminary evidence-based practice scheme for insulin injection. Results The evidence-based practice of insulin injection in diabetic patients mainly included 4 aspects: establish a multidisciplinary team mainly consisted of diabetes specialist nurses;formulate a Best Practice Manual for Insulin Injection;build insulin injection processes;organize clinical nurses to receive relevant training and take clinical intervention measures. In addition, the Best Practice Manual involved 17 recommendations on 5 aspects, namely insulin injection instructors, treatment and procedures, education and counseling, case management, and monitoring.Conclusion The practice scheme is constructed in a scientific manner and worthy of further clinical application and promotion, providing a reference for the self-management of insulin injection in diabetic patients.
