摘要
目的:测算公立医院螺旋断层放射治疗(TOMO)设备临床使用成本,为合理制定价格政策提供依据。方法:采用自制调查表,对我国东部、中部和西部地区共5家公立医院TOMO及医用直线加速器实际应用成本进行调查。采用作业成本法(ABC),根据不同疾病的放射治疗流程及成本动因对各医院TOMO治疗鼻咽癌、宫颈癌进行成本测算和分析。结果:TOMO单次放射治疗成本约为5200元(2982.35~8401.5元),根据鼻咽癌和宫颈癌一个疗程放射治疗次数不同,两种癌症一个疗程所需成本为13~17万元不等,其中85%为耗材、设备折旧和维护成本。结论:各地医院放射治疗设备临床TOMO使用定价和支付政策差异较大,国家应对医用TOMO临床使用定价和支付标准做统一规范,并建立动态调整机制。
Objective:To analyze the cost of clinical use of tomotherapy(TOMO)equipment of public hospital so as to provide basis for reasonable formulating price policy.Methods:The self-made questionnaire was adopted to investigate the actual application cost of TOMO and linear accelerators of 5 public hospitals in eastern,middle and western regions of China.The activity-based costing was adopted to implement cost calculation and analysis for treating nasopharynx cancer and cervical cancer with TOMO of each hospital according to the radiotherapy process and cost driver.Results:The single radiotherapy cost of TOMO was CNY 5200(CNY 2982.35-8401.5).The total cost of one course of treatment of two kinds of carcinomas was 130-170 thousand CNY according to the different radiotherapy frequency of nasopharyngeal cancer and cervical cancer.The cost of consumables,depreciation and maintenance was 85%of total.Conclusion:There are larger differences in pricing and payment policy of using TOMO of radiotherapy equipment in hospital of various regions.National should implement unified regulation for pricing and payment standard of medical TOMO in clinical use,and establish dynamic adjustment mechanism.
