摘要
目的:利用生物资源库大数据分析广州地区成人过敏疾病患者的过敏原sIgE分布特点,为临床提供流行病学数据。方法:共纳入2016年1月~2018年12月就诊于本院拟诊断为过敏性疾病的成人(≥18岁)患者2005例,并采用ImmunoCAP系统检测本地区常见的8种过敏原sIgE。结果:广州地区成人过敏性疾病患者中屋尘螨(34.41%)和蟑螂(24.14%)为主要的吸入性过敏原,虾(21.00%)为主要的食物过敏原。屋尘螨、猫毛、艾蒿、蟑螂和虾过敏原阳性率随年龄增加呈下降趋势,男性阳性率略高于女性,其中蟑螂在30~39岁和≥60岁年龄段中差异具有统计学意义(39.77%vs 27.38%,19.49%vs 10.53%,P<0.05),在≥60岁年龄阶段烟曲霉菌和虾的性别差异具有统计学意义(10.00%vs 4.05%,19.49%vs 8.50%,P<0.05)。对于小麦和大豆,阳性率与年龄无明显线性关系,且其男女阳性率在各年龄段差异均无统计学意义(P>0.05)。鼻炎组、哮喘组、皮肤过敏组和其他过敏性疾病组的屋尘螨、猫毛、蟑螂和虾过敏原阳性率差异具有统计学意义(P<0.05),鼻炎组患者阳性率最高。屋尘螨阳性样本中超过70%的患者合并了其他7项过敏原中至少1项过敏原共同致敏,其中以合并蟑螂和虾过敏原为主;相关性分析结果表明屋尘螨、蟑螂和虾过敏原两两之间呈显著正相关关系(0.823:i6 vs f24;0.801:d1 vs f24;0.737:d1 vs i6;P<0.01)。结论:在成人过敏性疾病中,过敏原以屋尘螨最为常见,且患者常对多种过敏原同时过敏。研究不同过敏性疾病患者过敏原分布特征对过敏性疾病的预防诊治具有重要意义。
Objective:Using big data of biological resources database to analyze the distribution characteristics of allergen sIgE in native adult patients with allergic diseases,to provide epidemiological data for clinicians.Methods:A total of 2005 adult patients(≥18 years old)with allergic diseases were admitted to our hospital from January 2016 to December 2018.ImmunoCAP system was used to detect 8 kinds of allergen sIgE in this area.Results:House dust mites(34.41%)and cockroaches(24.14%)was the main inhaled allergens,and shrimp(21.00%)was the main food allergens in adults with allergic diseases in Guangzhou.The house dust mite,cat hair,mugwort,cockroach and shrimp allergen positive rates decreased with increasing age,and the male positive rate was slightly higher than that of females,and the difference of cockroach at 30-39 and≥60 age group was statistically significant.(39.77%vs 27.38%,19.49%vs 10.53%,P<0.05),for aspergillus fumigatus and shrimp,the gender difference was statistically significant at the age of 60 years(10.00%vs 4.05%,19.49%vs 8.50%,P<0.05).For wheat and soybeans,there were no significant linear relationship between positive rate and age,and there was no significant difference in male and female positive rate between all ages(P>0.05).The positive rates of house dust mite,cat hair,cockroach and shrimp allergens rhinitis group,asthma group,skin allergy group and other allergic diseases group were statistically significant difference(P<0.05),with the highest positive rate in patients with rhinitis.More than 70%of the patients with positive house dust mite samples combined at least one of the other seven allergens to cause common sensitization,especially combined with cockroach and shrimp allergens.And there was a high positive correlation between the allergens of house dust mites,cockroaches and shrimp(0.823:i6 vs f24;0.801:d1 vs f24;0.737:d1 vs i6,P<0.01).Conclusion:House dust mite is the most common allergen in adult allergic diseases,and patients are often allergic to many kinds of allergens at the same time.Study of the distribution characteristics of allergens in patients with different allergic diseases is of great significance for the prevention and treatment of allergic diseases and provides relevant information for clinical diagnosis.
