摘要
目的:探讨初治涂阳肺结核合并糖尿病患者胸部影像学特点。方法:2019年5月-2020年4月收治初治涂阳肺结核合并糖尿病患者50例,作为观察组;同时选取同期住院的单纯性初治涂阳肺结核患者50例,作为对照组;所有患者均拍摄胸部平片及CT胸部扫描检查。比较两组治疗前胸部X线表现、治疗后病灶吸收情况及血糖控制情况。结果:观察组胸部X线表现为大片状阴影比例及有空洞比例均明显高于对照组,差异有统计学意义(P<0.05)。观察组治疗2个月后血糖控制较好33例,其中病灶吸收好转31例,无变化2例,好转率为93.9%;血糖控制较差17例,好转11例,无变化4例,恶化2例,好转率为64.7%;对照组好转率为100.0%。结论:大片状阴影、空洞是初治涂阳肺结核合并糖尿病患者胸部影像学的主要特点,治疗效果与血糖控制效果有密切关系。
Objective:To investigate the characteristics of chest imaging in newly treated patients with primary smear positive pulmonary tuberculosis complicated with diabetes.Methods:From May 2019 to April 2020,50 cases of newly treated primary smear positive pulmonary tuberculosis with diabetes were enrolled as the observation group.50 cases of simply newly treated primary smear positive pulmonary tuberculosis in the hospital were enrolled as the control group at the same time.Chest radiographs and CT chest scans were performed in all patients.Before and after treatment,the chest radiograph findings,the absorption of lesions and the control of blood glucose were compared between two groups.Results:Large shadow proportion and void proportion of chest radiograph findings in the observation group were significantly higher than the control group,the differences were statistically significant(P<0.05).There were 33cases'blood glucose control were better in the observation group after 2 months of treatment,among them,31 cases had better absorption,2 cases had no change and the improvement rate was 93.9%.Blood glucose control was poor in 17 cases,improved in 11 cases,no change in 4 cases,deterioration in 2 cases,the improvement rate was 64.7%.The improvement rate was 100.0%in the control group.Conclusion:Large shadow and voids are the main features of chest imaging in newly treated patients with primary smear positive pulmonary tuberculosis complicated with diabetes mellitus,the therapeutic effect is closely related to the effect of blood glucose control.
