新型冠状病毒肺炎患者住院时间和病程的影响因素分析 被引量:20

Influencing factors of virus clearance of COVID-19 patients

摘要 目的分析影响新型冠状病毒肺炎(COVID-19)患者病毒清除的相关因素,为COVID-19的防治提供科学依据。方法选择2020年1月14日至3月9日深圳市出院的381例COVID-19患者。收集患者年龄、性别以及感染新型冠状病毒的时间、地点等基本情况,同时利用实时荧光定量PCR和化学发光法分别检测SARS-Co V-2核酸Ct值以及患者血清中SARS-Co V-2 Ig G抗体水平。分析患者基本情况、确诊时Ct值和血清抗体水平高低对患者住院时间和病程的影响。结果COVID-19患者年龄、发病时间与住院时间及病程呈正相关(P<0.05);60岁以上男性患者的住院时间显著长于60岁以上女性患者(P<0.05);1月份发病患者的住院时间、发病到入院的时间和病程均显著长于2月份发病患者(P<0.05)。不同感染地点和患者确诊时Ct值与住院时间和病程之间不存在相关性(P>0.05);发病第2周核酸已转阴患者的抗体水平显著高于核酸阳性患者的抗体水平(P<0.05)。结论COVID-19患者的年龄、性别、感染的时间段以及Ig G抗体出现的早晚与病程有密切的关系,年龄越大、男性患者、感染时间越早以及早期抗体水平低都会使病程延长。 Objective To analysis the related factors that influenced the elimination of virus in patients with coronavirus disease 2019(COVID-19),and help with its prevention and control.Methods 381 COVID-19 patients discharged from January 14 to March 9 in Shenzhen were included in the study.The viral nucleic acid and the level of serum IgG antibodies specific to SARS-CoV-2 were detected by qRT-PCR and automated chemiluminescence immunoassay,respectively.The influences of age,gender,infection time and sites,as well as viral nucleic acid Ct values and IgG level on length of stay(LOS)and course of disease(COD)of patients were evaluated by SPSS software.Results The age and infection time of COVID-19 patients was positively correlated with their LOS and COD(P<0.05).The LOS of senior male patients(>60 years old)was significantly longer than that of senior female patients(P<0.05).Patients with illness onset in January had significantly longer LOS,time from onset to admission and COD than those with onset in February(P<0.05).There were no differences in LOS and COD among patients with different infections sites and different Ct values at diagnosis(P>0.05).In the second week after illness onset,the IgG level of the viral RNA negative patients was obviously higher than those viral RNA positive patients(P<0.05).Conclusions The age,gender,infection time,and IgG levels of early stage were closely related to the course of disease.Prolonged course was associated with advanced age,male gender,earlier infection and lower IgG level in the early stage.
