S-Detect技术在甲状腺结节超声诊断中的初步应用 被引量:4

摘要 目的:探讨S-Detect技术在甲状腺结节超声诊断的临床价值。方法:选取292例患者,共321个结节应用S-Detect技术进行检查分类,将得到的结果与病理结果进行对比分析。结果:病理结果中良性病灶104个(32. 4%),恶性病灶217个(67. 6%)。以细胞学病理为金标准,S-Detect技术横切面及纵切面的诊断准确率分别为61. 22%(150/245)、66. 12%(162/245),两者的敏感性、特异性、阳性预测值、阴性预测值分别为68. 39%、48. 89%、69. 74%、47. 31%及74. 19%、52. 22%、72. 78%、54. 02%;以组织学病理为金标准,S-Detect技术横切面及纵切面诊断的准确率分别为69. 74%(53/76)、77. 63%(59/76),两者的敏感性、特异性、阳性预测值、阴性预测值分别为74. 19%、50. 00%、86. 79%、30. 43%及83. 87%、50. 00%、88. 14%、41. 18%。结论:S-Detect技术对于提高甲状腺结节诊断准确率、降低超声医生劳动负荷有积极意义,其使用规范及操作指南有待建立。 Objective:To explore the clinical value of S-Detect in the ultrasonic diagnosis of thyroid nodules.Methods:A total of 292 patients were enrolled,and 321 nodules were examined by S-Detect.The results obtained were compared with pathological results.Results:There were 104 benign lesions(32.4%)and 217 malignant lesions(67.6%).With cytopathology as gold standard,the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of S-Detect(transverse section vs longitudinal sections)were 61.22%(150/245)vs 66.12%(162/245),68.39%vs 74.19%,48.89%vs 52.22%,69.74%vs 72.78%,and 47.31%vs 54.02%,respectively;with histopathology as gold standard,the above-mentioned were 69.74%(53/76)vs 77.63%(59/76),74.19%vs 83.87%,50.00%vs 50.00%,86.79%vs 88.14%,and 30.43%vs 41.18%,respectively.Conclusion:S-Detect technology is positive in improving the accuracy of thyroid nodule diagnosis and reducing the burden of ultrasound doctors,and relevant specifications and operation guidelines are need to be established.
