摘要
目的 miR-92a及miR-224在宫颈癌中存在异常表达,参与宫颈癌的发生发展。本研究旨在探讨宫颈癌患者血清miR-92a及miR-224的表达对诊断和预后的评估价值。方法选取2015-01-01-2017-06-30海南西部中心医院收治的148例宫颈癌患者临床资料和对照组健康体检正常女性60名。实时荧光定量PCR技术检测宫颈癌患者血清miR-92a及miR-224表达水平。以miR-92a及miR-224的中位数为标准将宫颈癌患者分为高miR-92a及miR-224组和低miR-92a及miR-224组,对2组临床病理特征进行比较。应用Kaplan-Meier法绘制生存曲线,单因素及多因素COX回归模型分析影响宫颈癌患者预后的不良危险因素。结果宫颈癌组血清miR-92a(3.26±1.28)及miR-224(6.12±2.15)表达水平高于对照组(0.72±0.31、1.35±0.64),差异有统计学意义,t值分别为10.905和13.216,均P<0.01。miR-92a及miR-224高表达与宫颈癌患者的临床分期、浸润深度、淋巴结转移及脉管浸润有关,均P<0.05。KaplanMeier分析与Log-rank检验结果显示,miR-92a及miR-224高表达与宫颈癌患者生存期短有关,均P<0.01。多因素COX回归模型分析显示,浸润深度〔HR(95%CI)=2.153(1.437~3.526)〕、淋巴结转移〔HR(95%CI)=3.115(2.367~5.218)〕、miR-92a≥1.82〔HR(95%CI)=3.502(2.864~6.273)〕及miR-224≥3.63〔HR(95%CI)=4.184(3.506~10.207)〕是影响宫颈癌患者预后不良的独立危险因素。结论 miR-92a及miR-224高表达与宫颈癌患者生存期短有关,可作为预测宫颈癌预后不良的生物学标志物。
OBJECTIVE The abnormal expression of miR-92 aand miR-224 in cervical cancer is involved in the occurrence and development of cervical cancer.The purpose of this study was to investigate the expression of serum miR-92 aand miR-224 in patients with cervical cancer and their diagnostic and prognostic value.METHODS From January 1,2015 to June 30,2017,148 patients with cervical cancer and 60 normal women in the control group were selected from Hainan Western Central Hospital.The expression levels of serum miR-92 aand miR-224 in cervical cancer patients were detected by real-time fluorescence quantitative PCR.The truncation values of miR-92 aand miR-224 were determined by ROC curve,and cervical cancer patients were divided into high miR-92 aand miR-224 groups and low miR-92 aand miR-224 groups.The clinicopathological characteristics of the two groups were compared.Kaplan-meier method was used to draw the survival curve,and univariate and multivariate COX regression models were used to analyze the risk factors affecting poor prognosis of patients with cervical cancer.RESULTS The expression level of miR-92 a(3.26±1.28)and miR-224(6.12±2.15)in cervical cancer group was higher than that in control group(0.72±0.31,1.35±0.64),the difference was statistically significant(t=10.905,13.216,P<0.01).The high expression of miR-92 aand miR-224 was associated with clinical stage,depth of invasion,lymph node metastasis and vascular invasion of cervical cancer(all P<0.05).The results of Kaplan-Meier analysis and Log-rank test showed that the high expression of miR-92 aand miR-224 was related to the short survival time of patients with cervical cancer(both P<0.01).Multivariate COX regression analysis showed that invasion depth[HR(95%CI)=2.153(1.437-3.526)],lymph node metastasis[HR(95%CI)=3.115(2.367-5.218)],miR-92 a≥1.82[HR(95%CI)=3.502(2.864-6.273)]and miR-224≥3.63[HR(95%CI)=4.184(3.506-10.207)]were independent risk factors for poor prognosis of cervical cancer patients.CONCLUSION The high expression of miR-92 aand miR-224 is associated with the short survival period of patients with cervical cancer,which can be used as biological markers to predict the poor prognosis of cervical cancer.
