摘要
目的探讨多层螺旋CT联合肌骨超声诊断新兵急性踝关节训练伤的临床价值。方法总结分析2018年12月~2019年6月78例急性踝关节训练伤的新兵,均于伤后24 h内接受多层螺旋CT联合肌骨超声检查。结果 78例急性踝关节损伤新兵中,骨折25例(8例为旋后外旋型,4例为旋前外旋型,6例为旋后内收型,4例为旋前外展型,3例为旋前背屈型);骨挫伤10例;骨软骨损伤8例;韧带损伤29例(三角韧带损伤11例,距腓前韧带损伤8例,跟腓韧带损伤5例,下胫腓前韧带损伤5例);肌腱损伤6例。多层螺旋CT三维重建联合肌骨超声检查诊断准确率100%(78/78)显著高于多层螺旋CT三维重建检查诊断82.05%(64/78)(P <0.05)。结论多层螺旋CT联合肌骨超声诊断新兵急性踝关节训练伤,诊断率高,值得临床推广。
Objective To explore the clinical value of multi-slice spiral computed tomography(MSCT) combined with musculoskeletal ultrasound(MUSU) in the diagnosis of the training-induced ankle joint injury in recruits. Methods The clinical characteristics of 78 recruits with training-induced acute ankle joint injury in our hospital from December 2018 to June 2019 were summarized and analyzed, MSCT combined with MUSU was performed for all the recruits 24 hours after injury. Results Among 78 cases of acute ankle joint injury, there were 25 cases of fractures(8 cases of supination-external rotation ankle fractures, 4 cases of pronation-external rotation ankle fractures, 6 cases of supination adduction ankle fractures, and 4 pronation abduction ankle fractures,and 3 cases of pronation dorsiflexion ankle fractures), 10 cases of bone contusion, 29 cases of ligament injury(11 cases of injury to the deltoid ligament, 8 cases of injury to the anterior talofibular ligament, 5 cases of injury to the calcaneofibular ligament, 5 cases of injury to the posterior-inferior tibiofibular ligament), and 6 cases of tendon injury. The diagnosis accuracy of 3 D reconstruction of MSCT combined with MUSU(78/78, 100%) was significantly higher than that of 3 D reconstruction of MSCT alone(64/78, 82.05%),showing a statistically significant difference(P < 0.05). Conclusion MSCT combined with MUSU is highly accurate in the diagnosis of the training-induced ankle joint injury in recruits, which deserves clinical application.
