摘要
目的:观察早期持续气道正压通气(CPAP)对重症肺炎合并呼吸衰竭患儿的血气指标变化,并分析预后情况。方法:将本院2017年2月~2020年2月接治的66例重症肺炎并呼吸衰竭患儿纳入研究中,基于乱数表法分成研究组和对照组,各33例;对照组行常规通气,研究组行CPAP,对比两组疗效,测定治疗前后血气指标变化,并掌握预后情况。结果:研究组临床症状消失时间、心率恢复正常时间及住院时间均显著短于对照组(P<0.05)。在治疗后测定,两组患儿的SaO2、PaCO2、PaO2及pH均有改善,但研究组更优于对照组,差异有统计学意义(P<0.05)。研究组并发症率、病死率均显著低于对照组(P<0.05)。结论:在重症肺炎并呼吸衰竭患儿临床治疗中进行早期CPAP,可有效改善症状和血气状态,实现良好预后。
Objective:To observe the effect of early continuous positive airway pressure(CPAP)on blood gas indexes in children with severe pneumonia complicated with respiratory failure,and analyze the prognosis.Method:66 children with severe pneumonia complicated with respiratory failure who were treated in our hospital from February 2017 to February 2020 were included in the study,and they were divided into study group and control group based on random number table method,33 cases in each group;the control group was given conventional ventilation,and the study group was given CPAP.The curative effect of the two groups was compared,the changes of blood gas indexes before and after treatment were measured,and the prognosis was mastered.Result:The clinical symptoms disappeared time,heart rate returned to normal time and hospital stay in the study group were significantly shorter than those in the control group(P<0.05).After treatment,SaO2,PaCO2,PaO2 and pH of the two groups were improved,but the study group was better than the control group,the difference was statistically significant(P<0.05).The complication rate and mortality rate of the study group were significantly lower than those of the control group(P<0.05).Conclusion:In the clinical treatment of children with severe pneumonia and respiratory failure,early CPAP can effectively improve the symptoms and blood gas status,and achieve a good prognosis.
