摘要
目的探讨腰骶部骨性参数对退变性腰椎滑脱(DLS)发生的意义。方法分析2015年1月~2018年3月在我院经治的105例L5退变性腰椎滑脱患者的临床影像资料,男47例,女58例,年龄(58.49±9.61)岁,组成退变性滑脱组(DLS组)。同时选取与退变性滑脱组年龄、性别条件相近的同期105例非滑脱退变性腰椎疾病患者作为对照组(非DLS组)。在站立位脊柱侧位X线片上进行矢状面参数的测量,包括骨盆倾斜角(PT)、骨盆入射角(PI)、骶骨倾斜角(SS)、L5椎体相对倾斜角(L5RI)、腰骶角(LA)、腰椎前凸角(LL)。结果 DLS组的PT(23.30±5.90)°,PI(56.82±10.21)°,SS(32.65±8.52)°,L5RI(19.25±6.46)°,LA(12.71±4.15)°,LL(26.53±9.34)°与非DLS组的参数比较,DLS组的PI明显更大(P<0.001),SS较大(P<0.001),L5RI明显更大(P<0.001),LL略大(P<0.001),PT较大(P>0.05),LA较小(P>0.05)。结论 DLS患者较同龄无腰椎滑脱的人群有更大的PI,SS,L5RI,LL,因此推测,较大的PI,SS,L5RI,LL可能是DLS发生的易感因素。
Objective To investigate the significance of lumbosacral osseous parameters in the occurrence of degenerative lumbar spondylolisthesis.Methods From January 2015 to March 2018,the imaging data of 105 patients with L5 degenerative lumbar spondylolisthesis treated in our hospital were analyzed.There were 47 males and 58 females enrolled,aged(58.49±9.61) years,which was composed of degenerative lumbar spondylolisthesis group(DLS group).In addition,105 patients with non-degenerative lumbar spondylolisthesis were selected as control group(non-DLS group),whose age and sex conditions were similar to those of degenerative lumbar spondylolisthesis group(DLS group).The spinopelvic sagittal parameters including pelvic tilt,pelvic incidence,sacral slope,relative inclination of L5 vertebral body,lumbosacral angle,lumbar lordosis were measured in standard lateral spine radiographs.Results In the DLS group,the pelvic tilt angle(PT) was(23.30±5.90)°,the incidence angle of pelvis(PI) was(56.82±10.21)°,the sacral slope(SS) was(32.65±8.52)°,the relative inclination of L5 vertebral body(L5RI) was(19.25±6.46)°,the lumbosacral angle(LA) was(12.71±4.15)°,the lumbar lordosis(LL) was(26.53±9.34)°.Compared with the parameters of non-DLS group,the PI in DLS group was significantly larger(P<0.001),the SS was larger(P<0.001),and the L5RI was significantly larger(P<0.001),the LL was slightly larger(P<0.001),the PT was larger(P=0.415),and the LA was smaller(P>0.05).Conclision DLS patients have larger PI,SS,LL,L5RI than those without lumbar spondylolisthesis,which maybe susceptible factors for the occurrence of DLS.
