摘要
目的:探讨MSCT对胃平滑肌瘤与胃神经鞘瘤的鉴别诊断价值。方法:回顾性分析经病理与免疫组化证实的胃平滑肌瘤、胃神经鞘瘤各16例患者的MSCT资料,对肿瘤的部位、长径、短径、长径/短径、生长方式、形态、溃疡、周围有无淋巴结、密度是否均匀、CT扫描各期CT值以及患者临床资料进行独立评估。结果:胃平滑肌瘤与胃神经鞘瘤在年龄、发生部位、长径/短径、生长方式、周围有无淋巴结、静脉期CT值、平衡期CT值差异有统计学意义(P<0.05)。ROC曲线显示,当患者年龄<59岁时,其诊断胃平滑肌瘤的敏感度、特异度分别为43.8%(7/16)、100.0%(16/16);当肿瘤长径/短径>1.4时,其诊断胃平滑肌瘤的敏感度、特异度分别为87.5%(14/16)、87.5%(14/16);当静脉期CT值<65 HU时,其诊断胃平滑肌瘤的敏感度、特异度分别为75.0%(12/16)、100.0%(16/16);当平衡期CT值<77 HU时,其诊断胃平滑肌瘤敏感度、特异度分别为81.3%(13/16)、100.0%(16/16)。结论:MSCT对胃平滑肌瘤与胃神经鞘瘤鉴别诊断具有很高的价值。
Objective:To investigate the value of MSCT in the differential diagnosis of gastric leiomyoma and gastric schwannoma.Methods:The MSCT data of 16 cases of gastric leiomyoma and 16 cases of gastric schwannoma were retrospectively analyzed.All of the tumors were confirmed by pathology and immunohistochemistry.The tumor location,long diameter(LD),short diameter(SD),LD/SD,growth pattern,shape,ulcer,surrounding lymph nodes,uniform density,CT values of each phase of CT scan and clinical data of the two groups were independently evaluated.Results:Gastric leiomyoma and gastric schwannoma in age,location,LD/SD,growth pattern,surrounding lymph nodes,CT values in venous phase and equilibrium phase was statistically significant(P<0.05).The ROC curve showed that when the cutoff value of age<59 years,the sensitivity and specificity for diagnosing gastric leiomyoma were 43.8%(7/16)and 100.0%(16/16),respectively.When the LD/SD of the tumor>1.4,the sensitivity and specificity for diagnosing gastric leiomyoma were 87.5%(14/16)and 87.5%(14/16),respectively.When CT value<65 HU in venous phase,the sensitivity and specificity for diagnosing gastric leiomyoma were 75.0%(12/16)and 100.0%(16/16),respectively.When CT value<77 HU in equilibrium phase,the sensitivity and specificity for diagnosing gastric leiomyoma were 81.3%(13/16)and 100.0%(16/16),respectively.Conclusion:MSCT has high value in the differential diagnosis of gastric leiomyoma and gastric schwannoma.
