摘要
目的探讨糖皮质激素联合百令胶囊治疗Graves病(GD)的疗效及安全性。方法选择2017年3月-2019年3月我院收治的GD患者130例为研究对象,根据治疗方式分为观察组和对照组,每组65例,对照组给予泼尼松龙片治疗;观察组在对照组基础上加用百令胶囊。连续治疗3个月后观察2组甲状腺肿疗效、治疗前后甲状腺激素、甲状腺抗体、细胞免疫功能变化,比较2组不良反应及1年复发率。结果观察组甲状腺肿大治疗总有效率显著高于对照组(P<0.05);与治疗前比较,2组治疗后游离三碘甲状腺原氨酸、游离四碘甲状腺原氨酸、促甲状腺素受体抗体、甲状腺球蛋白抗体、甲状腺过氧化物酶抗体水平、CD4+、CD4+/CD8+均显著降低,且观察组低于对照组(P<0.05)。与治疗前比较,2组促甲状腺激素、CD8+水平均显著升高,且观察组高于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05);观察组1年复发率显著低于对照组(P<0.05)。结论糖皮质激素联合百令胶囊治疗GD疗效确切,可有效改善患者甲状腺功能,减轻自身免疫反应,降低复发率。
Objective To explore the efficacy and safety of glucocorticoid combined with Bailing capsule in the treatment of Graves'disease(GD).Methods A total of 130 cases of GD patients treated in our hospital from March 2017 to March 2019 were selected as the research subjects,and they were divided into observation group(n=65)and control group(n=65)according to the treatment methods.The control group was given prednisone tablets,while the observation group was given Bailing Capsule on the basis of the control group.After 3 months of continuous treatment,the curative effect of goiter,and the changes of thyroid hormone,thyroid antibody and cellular immune function before and after treatment were observed,and the adverse reactions and 1-year recurrence rate of the two groups were compared.Results The total effective rate of goiter treatment in the observation group was significantly higher than that in the control group(P<0.05).Compared with those before treatment,the levels of free triiodothyronine,free tetraiodothyronine,thyrotropin receptor antibody,thyroglobulin antibody,thyroid peroxidase antibody,CD4+,CD4+/CD8+in the observation group were significantly lower than those in the control group(P<0.05).Compared with those before treatment,the levels of thyroid stimulating hormone and CD8+in the two groups were significantly increased,and the levels were higher in the observation group than in the control group C(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05),and the 1-year recurrence rate of the observation group was significantly lower than that of the control group(P<0.05).Conclusion Glucocorticoid combined with Bailing capsule has exact efficacy in the treatment of GD,which can effectively improve thyroid function,relieve autoimmune response and reduce recurrence rate.
