摘要
目的:探讨为进行功能性鼻内镜鼻窦手术的患者使用右美托咪定辅助实施局部麻醉对其术中的出血量及术野质量的影响。方法:选取2017年1月至2018年12月期间在江苏省南通市通州区人民医院进行功能性鼻内镜鼻窦手术的142例患者作为研究对象。随机将这些患者分为观察组和对照组。为两组患者均使用丁卡因联合利多卡因进行局部麻醉。在此基础上,为观察组患者使用右美托咪定辅助进行局部麻醉。然后观察两组患者术中的心率、平均动脉压、出血量、术野质量、手术耗时及医疗费用。结果:观察组患者术中的出血量少于对照组患者,其术野质量评分低于对照组患者,其医疗费用少于对照组患者,P<0.05。两组患者的手术耗时、术中不同时间的心率及平均动脉压相比,P>0.05。结论:为进行功能性鼻内镜鼻窦手术的患者使用右美托咪定辅助实施局部麻醉可减少其术中的出血量,提高其术野的质量,减少其医疗费用,且不会对其术中血流动力学的稳定性造成影响。
Objective:To investigate the effect of dexmedetomidine assisted local anesthesia on the amount of bleeding and the quality of operative field in patients undergoing functional endoscopic sinus surgery.Methods:142 patients who underwent functional endoscopic sinus surgery in Tongzhou District People’s Hospital of Nantong city,Jiangsu Province from January 2017 to December 2018 were selected as study subjects.These patients were randomly divided into observation group and control group.Both groups were given local anesthesia with tetracaine combined with lidocaine.On this basis,dexmedetomidine was used as an assistant for local anesthesia in the observation group.Then the heart rate,mean arterial pressure,blood loss,field quality,operation time,and medical expenses were observed.Results:In the observation group,the amount of bleeding was less than that in the control group,the quality score of operative field was lower than that in the control group,and the medical expenses were less than that in the control group,P<0.05.Compared with the operation time,heart rate and mean arterial pressure at different times during the operation,P>0.05 was obtained.Conclusion:Dexmedetomidine assisted local anesthesia for patients undergoing functional endoscopic sinus surgery can reduce intraoperative blood loss,improve the quality of surgical field,and reduce medical costs,without affecting the stability of intraoperative hemodynamics.
