摘要
目的观察通络益气方联合椎管内阻滞治疗带状疱疹后遗神经痛(PHN)的临床疗效及其对血清炎症因子、焦虑抑郁情绪的影响。方法选取厦门大学附属第一医院收治的110例顽固性PHN患者作为研究对象,按随机数字表法分为观察组和对照组,每组55例。对照组给予椎管内阻滞疗法,观察组给予通络益气方联合椎管内阻滞疗法。观察两组患者治疗后临床疗效,治疗前后中医证候积分、疼痛评分(VAS)、焦虑评分(SAS)、抑郁评分(SDS)评分及其血清炎症因子水平、CD4^+、CD4^+/CD8^+、Treg,Th17水平。结果观察组治疗后中医证候积分、VAS、SDS、SAS评分、血清炎症因子(IL-6、IL-2、TNF-α、INF-γ)水平、Treg,Th17均低于对照组,CD4^+、CD4^+/CD8^+水平高于对照组;观察组治疗后总有效率明显高于对照组(P<0.05),差异均有统计学意义。结论通络益气方联合椎管内阻滞治疗PHN可有效降低血清炎症因子水平,改善机体免疫功能,减轻疼痛程度,减轻抑郁焦虑情绪,提高临床治疗效果。
Objective To observe the clinical efficacy of Tongluo Yiqi Decoction combined with spinal canal block in the treatment of postherpetic neuralgia(PHN) and its effect on serum inflammatory factors,anxiety and depression.Methods A total of 110 patients with intractable PHN admitted to the First Affiliated Hospital of Xiamen University were selected as the research objects.They were divided into observation group and control group according to the random number table method,with 55 cases in each group.The control group was given internal obstruction of the spinal canal,while the observational group was given Tongluo Yiqi prescription combined with internal obstruction of the spinal canal.The TCM symptom score,the VAS score,the SDS score,the SAS score,the level of serum inflammatory factors,CD4^+,CD4^+/CD8^+,Treg,Th17 were observed of two groups.Results After treatment in the observation group,the scores of TCM syndromes,VAS,SDS,SAS scores,serum inflammatory factors(IL-6,IL-2,TNF-α,INF-γ) levels,Treg and Th17 were all lower than those of the control group,and CD4^+The level of CD4^+/CD8^+was higher than that of the control group,and the total effective rate of the observation group was significantly higher than that of the control group(P<0.05),and the differences were statistically significant.Conclusion Tongluo Yiqi Decoction combined with intraspinal block in the treatment of PHN can effectively reduce the level of serum inflammatory factors,improve the immune function of the body,reduce the degree of pain,reduce depression and anxiety,and improve the clinical treatment effect.
