某院新型冠状病毒肺炎患儿输血治疗回顾性分析 被引量:2

Blood transfusions in pediatric patients with COVID-19 in Wuhan:a single hospital’s retrospective analysis

摘要 目的回顾性分析临床输血在新型冠状病毒肺炎患儿治疗中的应用情况。方法查阅2020年1月27日-3月15日在华中科技大学同济医学院附属武汉儿童医院(武汉市妇幼保健院)住院治疗的新型冠状病毒肺炎确诊患儿的电子病历,收集患儿的人口统计学数据、临床输血资料、实验室结果、胸部X光片结果以及治疗情况包括疾病临床分级、并发症、住院天数,预后等信息。结果在500例患儿中,8例接受了69次血液制剂输注,输血率为1.6%(8/500);血液制剂包括新鲜冰冻血浆63.5 U(含恢复期血浆3 U)、悬浮红细胞38 U、血小板14.5治疗量以及冷沉淀6 U;在10例重症及危重症患儿中,7例接受了血液制剂的输注,重症输血率为70.0%(7/10),所有危重症的患儿都接受了1次以上的输血治疗;血浆置换治疗可以显著降低危重症患儿的血浆细胞因子和炎症因子水平;在所有输血治疗中,未发生输血不良反应。结论这是武汉地区首篇关于临床输血治疗新型冠状病毒肺炎儿科患者的报道,新型冠状病毒肺炎患儿总体输血率与一般儿科呼吸系统疾病相近,使用最多的血液制剂是新鲜冰冻血浆,血浆置换可以降低细胞因子风暴及炎症反应因子水平,可以用于危重症患者的治疗。 Objective To retrospectively analyze the data about practical blood use in the pediatric patients with COVID-19. Methods The electronic medical records of pediatric patients with COVID-19 who were treated at Wuhan Children’s Hospital from January 27 to March 15, 2020 were reviewed and analyzed in terms of.demographic data, blood transfusion data, laboratory findings, chest imaging results and clinical treatments including clinical grading, complications and hospitalization days and prognosis.Results Of the 500 infected children, 8 received 69 occasions of blood transfusion, with an incidence of blood transfusion at 1.6%(8/500), consisting of fresh frozen plasma in 63.5 U(including convalescent plasma in 3 U), red blood cells in 38 U, platelet in 14.5 therapeutic dose and cryoprecipitate in 6 U. Of the 10 critically ill patients, 7 received blood transfusion more than one occasions, with an incidence of blood transfusion at 70.0%(7/10). Plasma exchange treatment could significantly reduce the levels of plasma cytokines and inflammatory factors in critically ill children.No adverse effects to blood transfusion was observed during all blood transfusion treatments. Conclusion No retrospective analysis regarding blood transfusion in infected children in Wuhan has been reported up to now. The overall incidence of blood transfusion in pediatric patients with COVID-19 is similar to that of those with general pediatric respiratory diseases. The most frequently used blood product is fresh frozen plasma, and plasma exchange, which can reduce the levels of cytokine storm and inflammatory response factor, is recommended for the treatment of critically ill children with COVID-19.
