摘要
目的探究陈士铎治疗儿科疾病的处方用药特色以及规律。方法从《陈士铎医学全书》中手工检索并且阅读关于儿科的医案,通过全文阅读提取方药信息,用Microsoft Excel建立"陈士铎儿科用药数据库",同时整理归纳数据,应用Excel 2013频数分析、以及结合专业知识分析归纳四气、五味和归经;应用IBM SPSS Statistics 19系统聚类分析;应用IBM SPSS Modeler 18.0药物间网络化关联展示以及关联规则分析。结果纳入29个方剂,总计82味药,用药频率为281次,使用频数≥10次的药物8味,人参、当归、白术、荆芥、茯苓、甘草、陈皮、麦冬,累计使用110频次,贡献百分比为39.15%,分析发现其中补气药、健脾药使用频率较高;四气以寒33频次(40.24%)、温29频次(35.37%)、平为主13频次(15.85%);五味以甘40频次(30.77)、辛38频次(29.23%)、苦30频次(23.08%)为主;归经药物主要入脾(胃)52频次(30.23%)、肺(大肠)38频次(22.09%)。系统聚类分析结果显示,陈氏多用健脾益气之药,多围绕四君子汤加减变化,同时消补兼施、消散并用;关联规则聚类分析结果显示,陈氏儿科疾病用药以四君子配伍消导之法最为明显。结论陈氏治疗儿科疾病,注重小儿易虚之体,用药多以脾胃入手;药物组方精当,补中有散,补泻兼施,注重五行生克制化关系用药,强调治法与选药。
Objective To explore the characteristics and rules of CHEN Shiduo’s prescription medication for treating pediatric diseases.Methods We manually retrieved and read medical records on pediatrics from CHEN Shiduo Medical Complete Book.And then we extracted prescription information through full text reading and used Microsoft Excel to establish"CHEN Shiduo Pediatric Medication Database".At the same time,we organized and summarized the data,applied Excel 2013 for frequency analysis.And combined with professional knowledge analysis,we summarized the four natures,five flavors and the meridians.IBM SPSS Statistics 19 was used for system cluster analysis and IBM SPSS Modeler 18.0 for networked drug association display and association rule analysis.Results There were 29 prescriptions in total,a total of 82 drugs,with a frequency of281 times,and 8 drugs with a frequency of more than 10 times.Renshen(Ginseng),Danggui(Angelica),Baizhu(Atractylodes),Jingjie(Nepeta),Fuling(Poria),Gancao(Licorice),Chenpi(Tangerine peel)and Maidong(Ophiopogon japonicus)were used 110 times in total.The contribution percentage was 39.15%,and the analysis found that the Qi-tonifying and spleen strengthening drugs were used more frequently.Thefrequency of cold nature was 33 times(40.24%),the frequency of warm nature 29 times(35.37%)and the frequency of gentle nature 13 times(15.85%).Thefrequency of sweet tastewas40 times(30.77),the frequency of pungent taste 38 times(29.23%)and the frequency of bitter taste 30 times(23.08%).Thefrequency of spleen(stomach)meridians was 52 times(30.23%),and the lung(large intestine)meridians 38 times(22.09%).The results of systematic cluster analysis showed that CHEN’s use of medicines for strengtheningspleen and Qi is mostly based on Sijunzi decoction,while simultaneously supplementing and dissipating.The clustering analysis results of association rules showed that Sijunzi decoction with purging therapy was most obvious.Conclusion CHEN’s treatment of pediatric diseases pays attention to the children’s susceptibility to deficiency,and the medication is mainly given to the spleen and stomach.The medication is well-prepared,supplemented with tonifying and dispersing and supplemented with purging.It focuses on the five elements.It emphasizes treatment and drug selection.
