摘要
目的探讨甲状腺转录因子1(TTF-1)、天冬氨酸蛋白酶A(Napsin A)、尾型同源框转录因子2(CDx2)、细胞角蛋白20(CK20)及肝肠黏着蛋白17(CDH17)在肺肠型腺癌(SPEA)的诊断价值。方法选择非小细胞肺癌患者(A组)、肠腺癌肺转移患者(B组)及SPEA患者(C组)各50例,采用免疫组化法检测肿瘤组织TTF-1、Napsin A、CDx2、CK20、CDH17蛋白表达,ROC曲线分析单项蛋白检测和联合检测对SPEA的诊断效能。结果A组TTF-1和Napsin A蛋白阳性表达率高于CDx2、CK20及CDH17(P<0.01);B组CDx2、CK20及CDH17蛋白阳性表达率高于TTF-1和Napsin A蛋白(P<0.01)。联合检测的AUC大于单项蛋白检测(P<0.01),联合检测诊断SPEA的灵敏度、特异度、阳性预测值及阴性预测值均高于单项蛋白检测(P<0.05或P<0.01)。结论TTF-1、Napsin A、CDx2、CK20和CDH17联合检测能更好诊断SPEA。
Objective To explore the value of thyroid transcription factor 1(TTF-1),Napsin A,caudal type homeobox transcription factor 2(CDx2),cytokeratin 20(CK20),and liver-intestine cadherin 17(CDH17)in the diagnosis of secondary pulmonary enteric adenocarcinoma(SPEA).Methods The protein expressions of TTF-1,Napsin A,CDx2,CK20 and CDH17 in tumor samples were detected using immunohistochemistry in 50 patients with non-small cell lung carcinoma(group A),50 patients with lung metastasis from intestinal adenocarcinoma(group B)and 50 patients with SPEA(group C).A ROC curve was drawn to evaluate the diagnostic efficacy of single protein detection and combined detection for SPEA.Results The positive expression rates of TTF-1 and Napsin A were higher than those of CDx2,CK20 and CDH17 in group A(P<0.01).The positive expression rates of CDx2,CK20 and CDH17 were higher than those of TTF-1 and Napsin A in group B(P<0.01).The AUC of combined detection was greater than that of single protein detection(P<0.01).The sensitivity,specificity,positive predictive value and negative predictive value detected by combined detection of above proteins were all higher than those detected by single protein detection(P<0.05 or P<0.01).Conclusion Combined detection of TTF-1,Napsin A,CDx2,CK20 and CDH17 can better diagnose SPEA.
