摘要
目的借助结构方程模型,寻找影响社区居民做出家庭医生签约决策的关键因素。方法 2018年5月4日-5月25日,对上海市浦东新区塘桥社区签约和未签约居民共201人进行签约影响因素调查,从中提取主观印象、需求、供给和就医体验四个潜变量,信度系数均大于0.700。结果主观印象和就医体验对签约倾向的路径系数,需求和签约倾向对签约服务利用情况的路径系数,估计值有统计学意义。需求和签约倾向对签约服务利用情况有直接作用,影响程度分别为0.347和0.533;供给、主观印象和就医体验对签约服务利用情况的影响不显著。结论需求才是关键因素,应充分挖掘辖区居民健康需求并提供相应的签约服务包。
OBJECTIVE To study the key factors influencing factors on community residents’ contract decision-making by means of structural equation modeling(SEM). METHODS From May 4, 2018 to May 25, 2018, a survey was conducted on the contract influencing factors of 201 contracted and contracted residents in the Tangqiao Community in Pudong New Area, Shanghai. Latent variables such as subjective impression, demand, supply and medical experience, were extracted with reliability analysis, exploratory factor analysis and confirmatory factor analysis, and the reliability coefficients of each latent variable were greater than 0.700. RESULTS The path coefficient of subjective impression and medical experience to contracting tendency, the path coefficient of demand and contracting tendency to contracted service utilization, were all statistical significance. Demand and contracting tendency had a direct impact on the contracted service utilization, the overall impact effect were 0.347 and 0.533 respectively. Supply, subjective impression and medical experience did not affect the contracted service utilization significantly. CONCLUSION Demand is the key factor. The CHS should fully tap the health needs of the area residents and provide the corresponding contract service package.
