摘要
目的探讨输尿管软镜钬激光内切开引流术治疗肾盂旁囊肿的疗效。方法回顾性分析2014年8月~2016年8月输尿管软镜钬激光内切开治疗肾盂旁囊肿19例资料。囊肿直径3.8~6.8 cm,平均4.9 cm。使用F8.5/9.9输尿管软镜,钬激光切开囊壁1~2 cm,留置F7输尿管支架管2个月。术后定期复查彩超或CT了解囊肿大小,询问症状缓解情况。结果19例均顺利完成手术,手术时间23~55 min,平均40 min,术中及术后无严重并发症发生。术后随访36~60个月,平均44.8月。术后3个月随访,13例肾盂旁囊肿消失,4例囊肿缩小>1/2,2例囊肿缩小<1/2。术后3年随访,腰痛均消失,16例囊肿消失,3例囊肿持续存在,但囊肿直径较术前缩小>1/2。随访期间无反复泌尿系感染,未见囊肿内结石形成。结论输尿管软镜钬激光内切开引流术治疗肾盂旁囊肿中期疗效确切,安全有效。
Objective To observe the mid-term clinical effect of flexible ureteroscopy combinded with the holmium laser for parapelvic renal cysts.Methods A retrospective study was conducted to analyze the clinical data of 19 patients with parapelvic cysts from August 2014 to August 2016.The diameters of the cysts were 3.8-6.8 cm(mean,4.9 cm).All the cases were treated by F8.5/9.9 flexible ureteroscope.The holmium laser was used to cut the wall of the cyst for 1-2 cm,and a F7 ureteral stent was retained for 2 months.After the operation,the size of the cyst was determined by color ultrasound or CT,and the symptom relief was inquired.Results All the cases were treated successfully without severe complications.The mean operation time was 40 min(range,23-55 min).The postoperative follow-ups lasted for 36-60 months(mean,44.8 months).After 3 months of operation,the renal cyst disappeared in 13 cases,and the size of 4 cases were reduced by more than half,and two cases were reduced by less than half.After 3 years follow-up,lumbago disappeared.Sixteen cysts disappeared,and 3 cysts continued to exist,but the diameter of cysts was more than 1/2 smaller than before.None of the patients had recurrent urinary tract infections and urolithiasis.Conclusion Flexible ureteroscopy combined with the holmium laser is safe with definite mid-term efficacy,and can be a treatment option for parapelvic renal cysts.
