摘要
目的比较腹膜后腹腔镜手术与开放手术治疗肾盂输尿管连接部梗阻(UPJO)的效果。方法回顾性分析216例UPJO患者的临床资料,其中113例接受腹膜后腹腔镜手术(A组),103例接受开放手术(B组),术式均为离断肾盂成形术。比较两组的手术时间、术中出血量、术后肠功能恢复所需时间、术后下床活动所需时间、术后住院时间、术后并发症发生情况及术后疗效。结果A组的术中出血量、术后肠功能恢复所需时间、术后下床活动所需时间、住院天数均少于或短于B组(均P<0.05);但两组的手术时间、术后并发症发生率、治疗有效率差异均无统计学意义(均P>0.05)。结论与开放手术相比,经腹膜后腹腔镜手术治疗UPJO具有出血少、损伤小、疼痛少、术后康复快等优点,且安全性相当。
Objective To compare the efficacy between retroperitoneal laparoscopic surgery and open surgery for the treatment of ureteropelvic junction obstruction(UPJO).Methods The clinical data of 216 patients with UPJO were analyzed retrospectively.Among them,113 cases received retroperitoneal laparoscopic surgery(group A),whereas 103 cases received open surgery(group B),and dismembered pyeloplasty was adopted in the surgery in both groups.Operation duration,intraoperative blood loss,time to postoperative bowel function recovery,postoperative ambulation time,postoperative hospital stay,incidence of postoperative complications,and postoperative efficacy were compared the between two groups.Results Group A had less intraoperative blood loss,shorter time to postoperative bowel function recovery,shorter postoperative ambulation time,and shorter postoperative hospital stay than group B(all P<0.05);however,there were no statistically significant differences between the two groups in operation duration,incidence rate of postoperative complications,or therapeutic inefficiency rate(all P>0.05).Conclusion Compared with open surgery,retroperitoneal laparoscopic surgery has advantages of less bleeding,minor trauma,less pain,and rapid recovery after surgery in the treatment of UPJO,achieving similar safety.
