摘要 目的探讨去甲肾上腺素(norepinephrine,NE)联合复合辅酶对脓毒症患者病情程度、氧化应激状态、免疫功能及炎症因子的影响。方法选取我院收治的114例脓毒症患者,分为对照组、NE组和NE联合复合辅酶组,每组各38例。比较3组患者治疗前及治疗后3、7 d的SOFA、APACHEⅡ评分,心肌损伤标志物(LDH、cTnI、BNP)、EF、氧化应激指标(SOD、GSH-Px、LHP、AOPP)、炎症因子(CRP、IL-6、IL-8、TNF-α)和免疫功能指标(CD4+、CD8+、CD4+/CD8+比值)水平。结果3组患者治疗后3、7 d的SOFA、APACHEⅡ评分均显著降低,心肌损伤标志物、EF、氧化应激指标、炎症因子和免疫功能指标均显著得到改善,且NE联合复合辅酶组改善程度显著优于NE组和对照组,NE组显著优于对照组,差异均具有统计学意义(P<0.05)。结论应用去NE联合复合辅酶治疗脓毒症,能够明显减轻患者病情,减轻患者全身炎症反应,改善患者氧化应激状态,提高患者免疫力。 Objective To explore to the effect of norepinephrine(NE)combined with coenzyme on the illness severity,oxidative stress,immune function and inflammatory factors in patients with sepsis.Methods 76 patients with sepsis admitted to our hospital were selected and divided into the control group,NE group and NE combined with coenzyme group according to the random number method,each group was 38 cases.The SOFA and APACHEII scores,levels of myocardial injury markers(LDH,cTnI,BNP),EF,oxidative stress indicators(SOD GSH-Px,LHP,AOPP)and immune function index(CD4+,CD8+,CD4+/CD8+ratio)were compared among 3 groups before and after treatment at 3 d,7 d.Results Scores of SOFA and APACHEII were significantly decreased,and levels of myocardial injury markers,EF,oxidative stress indicators and immune function index were significantly improved in 3 groups after treatment at 3 d,7 d,the extent of improvement in the NE combined with coenzyme group was significantly better than that in the NE group and control group,the extent of improvement in the NE group was significantly better than that in the control group,all above had statistical difference(P<0.05).Conclusion The treatment of norepinephrine combined with coenzyme on patients with sepsis could reduce illness severity of patients,systemic inflammatory response of patients,improve the oxidative stress state of patients,and improve the immune function of patients.
