摘要
目的分析窄带成像放大内镜在早期胃癌及癌前病变中的诊断价值。方法方便选取2018年1月-2019年1月期间该院接收的122例疑似早期胃癌患者为研究对象,依据检查方式不同划分为参照组(染色放大内镜)、研究组(摘掉成像放大内镜),每组61例,观察及统计两组患者病变轮廓、黏膜腺管开口形态、微血管形态清晰度、病理学诊断癌前病理结果及诊断效能情况。结果研究组微血管性形态、黏膜腺管开口形态、病变轮廓图片清晰度情况均较参照组略胜一筹,差异有统计学意义(t=9.900、12.191、6.939,P<0.05);组织病理学检查检出癌前病变、非癌前病变各为60例(49.18%)、62例(50.82%),则研究组特异度59(96.72%)、敏感度52(85.25%)及准确度54(88.52%),分别优于参照组特异度51(83.6%)、敏感度42(68.85%)及准确度46(75.41%),差异有统计学意义(χ^2=5.915、4.635,P<0.05)。结论窄带成像放大内镜用于早期胃癌及癌前病变中具有较高的应用价值。
Objective To analyze the diagnostic value of narrow band imaging in early gastric cancer and precancerous lesions.Methods Convenient select from January 2018 through January 2019 in the hospital during the period of receiving 122 suspected early gastric cancer patients as the research object,according to the inspection in a different way is divided into control group(dyeing magnification endoscopy),team imaging magnification endoscopy(take off),each group of 61 cases,observation and statistics of two groups of patients with lesions outline,mucosal gland openings form,microvascular form of clarity,pathology diagnosis precancerous pathological results and efficiency.Results The microvasculature morphology,the opening morphology of mucosal duct and the contour of lesion in the study group were better than those in the reference group,the difference was statistically significant(t=9.900,12.191,6.939,P<0.05).When 60 cases(49.18%)and 62 cases(50.82%)of precancerous and non-cancerous lesions were detected by histopathological examination,the specificity of the study group was 59(96.72%),sensitivity 52(85.25%)and accuracy 54(88.52%),which were better than that of the reference group 51(83.6%),sensitivity 42(68.85%)and accuracy 46(75.41%),the difference was statistically significant(χ^2=5.915,4.635,P<0.05).Conclusion Narrow band imaging magnifying endoscopy has high application value in early gastric cancer and precancerous lesions.
