摘要
目的运用系统评价的方法评价认知行为疗法(CBT)对癌症患者癌因性疲乏(CRF)症状的影响,为癌症患者开展CBT提供证据。方法计算机检索中国生物医学文献数据库(CBM)、万方数据库(Wanfang)、知网(CNKI)、维普(VIP)、PubMed、Embase、Cochrane Library从建库起到2019年8月关于CBT对CRF症状影响的随机对照试验(RCTs)。由2名研究者严格按照纳入、排除标准独立进行文献筛选、数据提取、质量评价,并应用RevMan 5.3软件进行Meta分析。结果共纳入16篇文献,总样本量1345例,Meta分析结果显示,CBT可以降低CRF评分[SMD=-1.08,95%CI(-1.54,-0.63),P<0.01]、疼痛评分[SMD=-1.32,95%CI(-2.23,-0.41),P<0.01]、睡眠评分[SMD=-0.87,95%CI(-1.48,-0.27),P<0.01]、焦虑评分[MD=-4.15,95%CI(-8.20,-0.11),P=0.04]、抑郁评分[MD=-7.03,95%CI(-8.71,-5.36),P<0.01],对恶心呕吐评分差异无统计学意义(P=0.32)。结论CBT可以改善癌症患者的CRF、疼痛、睡眠及焦虑、抑郁等身心症状,但对恶心呕吐的干预作用有待进一步的临床试验来验证。
Objective A systematic review was used to evaluate the effects of cognitive behavioral therapy on cancer-related fatigue in cancer patients,and to provide evidence for cognitive behavioral therapy in cancer patients.Methods Randomized controlled trials(RCTs)of the effect of cognitive behavioral therapy on cancer-related fatigue in these databases,including CBM,Wanfang Data,CNKI,VIP,PubMed,Embase,Cochrane Library,and were searched by computers from the establishment of the databases to August,2019.Two researchers independently conducted literature screening,data extraction,and quality evaluation in strict accordance with the inclusion and exclusion criteria,and applied RevMan 5.3 software for Meta analysis.Results A total of 16 RCTs with 1345 patients were included.Meta-analysis showed that cognitive behavioral therapy can reduce the fatigue score(SMD=-1.08,95%CI[-1.54,-0.63],P<0.01),pain score(SMD=-1.32,95%CI[-2.23,-0.41],P<0.01),sleep score(SMD=-0.87,95%CI[-1.48,-0.27],P<0.01),anxiety score(MD=-4.15,95%CI[-8.20,-0.11],P=0.04),depression score(MD=-7.03,95%CI[-8.71,-5.36],P<0.01).There was no significant difference in nausea and vomiting scores(P=0.32).Conclusion Cognitive behavioral therapy can improve the physical and mental symptoms such as cancer-related fatigue,pain,sleep,anxiety and depression in cancer patients,but the intervention effect on nausea and vomiting needs further confirmation.
