摘要
目的:探讨跟骨骨折患者应用药物及高压氧联合治疗对疼痛控制的影响及护理效果.方法:将本院收治的跟骨骨折患者纳入本项研究,选取2018年1月-2019年4月间69例跟骨骨折的患者,按照入院的先后顺序进行编号分组,其中1-34号为对照组,予以单纯西药治疗+常规护理;35-69号为观察组,在对照组的基础上进行高压氧辅助治疗及综合专科护理,在干预后进行效果评估,由接受过相关培训的骨科医生,使用视觉模拟评分、美国足踝外科协会踝与后足评分、简明健康状况调查表,分别对患者的疼痛程度、踝部功能以及生活质量进行评价评定,并调查2组血清5-羟色胺(5-hydroxytryptamine,5-HT)和P物质(Psubstance,PS)水平变化.结果:观察组治疗效果明显优于对照组,差异对比有意义(P<0.05);术前2组患者VAS评分以及5-HT、PS水平对比均无统计学意义(P>0.05),自术后3-7天,2组VAS评分较术前均有所降低,5-HT和PS水平较术前均有所升高;其中观察组VAS评分在术后5天、7天明显低于对照组,5-HT和PS水平在术后5天、7天明显高于对照组,差异对比有意义(P<0.05),关于SF-36评分,2组除BP得分较干预前有所降低之外,其他维度较前均有提高,组间对比发现观察组SF-36各维度均高于对照组,BP得分明显低于对照组,差异有统计学意义(P<0.05).结论:跟骨骨折患者应用药物及高压氧联合治疗效果显著,配合专科护理干预可有效控制疼痛程度,提高患者术后关节功能恢复,改善生活质量.
Objective:To explore the effect of medicine and hyperbaric oxygen combined therapy on pain control and nursing effect of calcaneus fracture patients.Methods:Patients with calcaneal fracture admitted to our hospital were included in this study,69 patients with calcaneal fracture from January 2018 to April 2019 were selected to be numbered and grouped according to the sequence of admission,among them,no.1-34 was the conventional group,which was treated with western medicine plus conventional nursing;no.35-69 was the intervention group,which was treated with hyperbaric oxygen assisted therapy and comprehensive specialized nursing on the basis of the conventional group,which was carried out after intervention.The results were evaluated by orthopedic doctors who had received relevant training,using visual simulation score,ankle and hind foot score of American Association of foot and ankle surgery and concise health status questionnaire.The pain degree,ankle function and quality of life of the patients were evaluated,and the changes of serum 5-hydroxytryptamine(5-HT)and Psubstance(PS)levels in the two groups were investigated.Results:The therapeutic effect of the intervention group was significantly better than that of the conventional group(P<0.05).Before operation,there was no significant difference in VAS score,5-HT and PS Level between the two groups(P>0.05).From 3 days to 7 days after operation,VAS score of the two groups was lower than that before operation,and 5-HT and PS levels were higher than that before operation.Among them,the VAS score of intervention group was significantly lower than that of routine group on the 5th and 7 th day after operation,and the levels of 5-HT and PS were significantly higher than that of routine group on the 5th and 7th day after operation,the difference was significant(P<0.05).With regard to SF-36 score,except BP score,the other dimensions of the two groups were higher than that of the former.The comparison between groups found that the SF-36 dimensions of intervention group were higher than that of routine group,and BP score was significantly lower than that of routine group,the difference was statistically significant(P<0.05).Conclusion:The combination of medicine and hyperbaric oxygen can effectively control the degree of pain,improve the recovery of joint function and improve the quality of life.
