摘要
目的探讨改善全膝关节置换术后早期膝关节功能恢复中采取围手术期系统康复治疗的影响。方法随机选取该院2015年1月—2019年1月实施全膝关节置换术的患者100例,随机均分为对照组和试验组各50例。对照组术后实施常规康复治疗,试验组采取围手术期系统康复治疗,比较两组手术前后疼痛评分、膝评分和膝功能评分,对比患者的并发症发生率和术后7 d伤口甲级愈合比例。结果手术前两组患者的疼痛评分、膝评分和膝功能评分均差异无统计学意义(P>0.05),术后各时点的评分均以试验组更优且组间差异有统计学意义(P<0.05)。试验组感染和下肢深静脉血栓发生率对比差异无统计学意义(P>0.05),术后7 d伤口甲级愈合率试验组的74.00%高于对照组的54.00%且差异有统计学意义(χ^2=4.340,P<0.05)。结论改善全膝关节置换术后早期膝关节功能恢复中采取围手术期系统康复治疗能更好地提升患者的膝关节功能,缩短患者的切口愈合时间,具有较高临床价值。
Objective To explore the effect of perioperative systemic rehabilitation in improving early knee function recovery after total knee replacement.Methods 100 patients undergoing total knee arthroplasty from January 2015 to January 2019 in the hospital were randomly selection divided into a control group and a test group of 50 patients each.The control group was given routine rehabilitation treatment after operation,and the experimental group was given systematic rehabilitation treatment during the perioperative period.The pain score,knee score,and knee function score were compared between the two groups before and after the operation.The incidence of complications and the proportion of wound healing at 7 days after surgery were compared.Results There was no statistically significant difference in pain score,knee score and knee function score between the two groups before surgery(P>0.05).The scores at each time point after surgery were better in the experimental group and the difference between the groups was statistically significant(P<0.05).There was no statistically significant difference between the incidence of infection and lower limb deep vein thrombosis in the test group(P>0.05).The wound healing rate of 7 d after surgery was 74.00%in the test group and 54.00%in the test group and the difference was statistically significant(χ^2=4.340,P<0.05).Conclusion The improvement of early knee function recovery after total knee arthroplasty by adopting perioperative systemic rehabilitation treatment can better improve the patient's knee function,shorten the incision healing time,and has a high clinical value.
