摘要
目的探讨皮下注射、胰岛素泵两种不同给药方式对妊娠期糖尿病(GDM)患者血糖达标时间、胰岛素用量及母婴并发症的影响。方法随机数表法将100例GDM患者分为两组,每组各50例,注射组使用多次皮下注射胰岛素治疗,胰岛素泵组使用胰岛素泵持续皮下给药治疗,均治疗4周。分析各组患者的血糖达标时间、胰岛素用量及母婴并发症的情况。结果胰岛素泵组血糖达标较注射组快,胰岛素用量比注射组少且有统计学差异(P<0.05),胰岛素泵组母体的各类并发症发生率均低于注射组(P<0.05),胰岛素组出生婴儿的各类并发症发生率都比注射组低,但只有新生儿呼吸窘迫综合征的差异具有统计学意义(P<0.05)。结论胰岛素泵在GDM治疗中能用更少的给药量达到优于皮下注射的血糖控制水平,并降低母婴双方并发症的发生率。
Objective To investigate the effects of subcutaneous injection and insulin pump on the blood glucose compliance time,insulin dosage and maternal and fetal complications in patients with gestational diabetes mellitus(GDM).Methods 100 patients with GDM were divided into 2 groups according to the random number table method,50 patients in each group.The injection group was treated with multiple subcutaneous injections of insulin,and the insulin pump group was treated with an insulin pump for subcutaneous administration for 4 weeks.The blood glucose compliance time,insulin dosage and maternal and child complications were analyzed in each group.Results The blood glucose level of the insulin pump group was faster than that of the injection group,and the insulin dosage was less than that of the injection group(P<0.05).The incidence of various complications of the insulin pump group was lower than that of the injection group(P<0.05).The incidence of various complications in the infants in the insulin group was lower than that in the injection group,but only the difference in neonatal respiratory distress syndrome was statistically significant(P<0.05).Conclusion Insulin pump can achieve better blood glucose control level than subcutaneous injection in GDM treatment,and reduce the incidence of complications between mother and infant.
