摘要
目的:从四个方面分析吻合器痔上黏膜环切术后肛门痉挛患者治疗方法的选择,为临床治疗本病提供新思路。方法:从“患者从未接触过中医”“对疼痛的畏惧心理较重”“提高瞬时疗效”“减少有创操作”方面考虑,选择针刺双侧次髎、会阳、秩边穴,中等刺激,留针30 min,另予蜡饼温敷腰骶部,且建议患者清淡饮食,避免久坐、久站等姿势,观察临床疗效。结果:3次治疗后,患者诉肛门痉挛好转,疼痛明显减轻,肛门坠胀感消失,大便后纸上无染血。继续治疗3次后,患者肛门痉挛等症状完全消失。1周后随访未再发。半年后随访亦未复发。结论:本病从西医手术、中医治病到患者生理、心理再到社会影响方面涉及问题较多,本文的治疗思路值得临床医师及治疗师参考。
Objective:From four aspects to analysis the choice of treatment methods for anal spasm patients after the procedure of prolapse and hemorrhoids,new ideas for clinical treatment of this disease are provided.Methods:From the aspects of"patients have never been exposed to traditional Chinese medicine","fear of pain is more serious","improve instantaneous curative effect",and"reduce invasive operation",we choose to acupuncture the Ciliao(次髎)Huiyang(会阳)and Zhibian(秩边)points on both sides,with moderate stimulation,keep the needle for 30 minutes,and warm the lumbosacral part with wax cake,meanwhile we suggest that patient should eat light food,avoid sitting for a long time,standing for a long time and other postures,to observe the clinical curative effect.Results:After 3 times of treatment,the anal spasm was improved,the pain was obviously relieved,the feeling of anal distention disappeared,and there was no blood on the paper after defecation.Continued 3 times treatment,the symptoms of anal spasm disappeared completely.There was no recurrence after one week.After half a year follow-up,there was no recurrence.Conclusion:This disease involves many problems from western medicine operation,traditional Chinese medicine treatments to patients'physiological,psychological and social influence.The treatment ideas are worth reference for clinicians and acupuncturists.
