摘要
老年性痴呆又称为阿尔茨海默病(Alzheimer Disease,AD),归属于中医“痴呆病”范畴。本文通过从八纲辨证的角度分析老年性痴呆的本质、邪正盛衰、疾病性质、病位深浅。阴证老年性痴呆临床上常见倦怠乏力、面色㿠白、畏寒肢冷等症状;阳证临床上常见眩晕头痛,心烦不寐,口干舌燥等症状。老年性痴呆多见里证,与表证无关,病位主要在脑,与心、肝、脾、肺、肾等关系甚为密切。寒证老年性痴呆多从脑髓失养,五脏气血亏耗立论;热证多从肝火、痰热、瘀热立论。老年性痴呆以虚证为多见,多由于其病程发展缓慢,呈进行性加重,病程一般较长,病久则虚,气血亏乏。但并非全无实证,由于老年性痴呆病程长,病机上常发生转化,日久形成瘀、痰、火、气等病理产物,因此疾病中后期常多见虚实夹杂之证。由于目前中医对老年性痴呆的辨证尚不规范、不统一,证候分型复杂多样,在一定程度上影响了对老年性痴呆的诊断及治疗。本文通过从八纲辨证的角度分析老年性痴呆,综合归纳老年性痴呆的常见病因、病位、病性、病机。其基本病机为病初起多属于阴证、虚证、寒证、里证,日久亦可兼夹阳证、热证、实证等标实证候。
Alzheimer Disease(AD),belongs to the category of"dementia"in traditional Chinese medicine.This paper analyzes the nature,the rise and fall of pathogenic factors,the nature and the depth of the disease of Alzheimer Disease from the perspective of eight categories of syndrome differentiation.The symptoms of senile dementia in Yin Syndrome are fatigue,pale complexion,chilly limbs,etc.vertigo and headache,restlessness,dry mouth and dry tongue are common in Yang syndrome.Alzheimer Disease is mainly located in brain,which is closely related to heart,liver,spleen,lung and kidney.Cold type Alzheimer Disease is mostly from the brain marrow loss of nourishment,five viscera Qi and blood depletion argument;heat type is mostly from liver fire,phlegm heat,blood stasis heat argumen.The most common form of Alzheimer Disease is deficiency syndrome,which is progressive aggravation due to the slow development of the course of disease.The course of disease is generally long,the disease lasts for a long time,and there is deficiency of Qi and blood.However,there is also excess.Due to the long course of Alzheimer Disease,the pathogenesis often changes,and the pathological products such as blood stasis,phlegm,fire and Qi are formed over time,so the syndrome of deficiency and excess are often seen in the middle and later stages of the disease.At present,the syndrome differentiation of Alzheimer Disease in traditional Chinese medicine is not standardized and unified,and the syndrome classification is complex and diverse,which to a certain extent affects the diagnosis and treatment of Alzheimer Disease.This paper analyzes Alzheimer Disease from the perspective of eight categories of syndrome differentiation,and summarizes the common causes,disease location,disease nature and pathogenesis of AD.The basic pathogenesis of the disease is mainly Yin Syndrome,deficiency syndrome,cold syndrome and internal syndrome,and it can also be combined with Yang Syndrome,heat syndrome and empirical syndrome for a long time.It is to provide some basis for the standardization of TCM treatment of Alzheimer Disease.
