摘要
目的:研究无创通气序贯治疗应用于慢性阻塞性肺疾病急性加重期(AECOPD)并呼吸衰竭中的临床价值。方法:选取汕头市第二人民医院呼吸内科2017年6月-2019年6月收治的60例AECOPD并呼吸衰竭患者为研究对象,按照随机数字表法将其分为试验组(30例)和对照组(30例)。对照组行常规正压通气支持治疗,试验组行无创通气序贯支持治疗。比较两组肺功能及呼吸机相关肺炎、气压伤、再插管发生情况。结果:治疗后试验组第一秒用力呼气容积(FEV1)、一秒用力呼气量占用力肺活量比值(FEV1/FVC)、FEV1占预计值百分比均显著高于对照组,差异有统计学意义(P<0.05)。试验组呼吸机相关肺炎、气压伤、再插管发生率均低于对照组,差异有统计学意义(P<0.05)。结论:无创通气序贯治疗AECOPD并呼吸衰竭,能有效改善患者肺功能,降低患者呼吸机相关疾病的发生。
Objective:To study the clinical value of non-invasive ventilation sequential therapy for AECOPD and respiratory failure.Method:A total of 60 patients with AECOPD and respiratory failure admitted in the department of respiratory medicine of Shantou Second People’s Hospital from June 2017 to June 2019 were selected as the study subjects,and they were divided into the experimental group(30 cases)and the control group(30 cases)according to the random number table method.The control group received conventional positive pressure ventilation support therapy,and the experimental group received non-invasive ventilation sequential support therapy.The pulmonary function,ventilator associated pneumonia,air pressure injury and re-intubation were compared between the two groups.Result:After treatment,the forced expiratory volume in the first second(FEV1),ratio of forced expiratory volume in the first second to forced vital capacity(FEV1/FVC),and percentage of FEV1 in the predicted value in the experimental group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The incidence of ventilator associated pneumonia,air pressure injury and re-intubation in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Non-invasive ventilation sequential support in the treatment of AECOPD and respiratory failure can effectively improve the patient’s pulmonary function and reduce the incidence of ventilator associated diseases.
