右美托咪定对全麻及体外循环下行心脏瓣膜置换术患者围术期炎症反应及脑氧摄取率的影响 被引量:3

摘要 目的分析全麻及体外循环(CPB)下心脏瓣膜置换术中应用右美托咪定对围术期炎症介质TNF-α、IL-6及脑氧摄取率(CERO2)的影响。方法纳入本院2017年12月至2019年12月收治的102例心脏瓣膜置换术患者,将其随机均分为试验组和参照组,各51例。麻醉诱导前10 min,参照组予以等速等量生理盐水,试验组予以0.5μg/kg右美托咪定。比较两组血流动力学指标、炎症反应指标、CERO2。结果T1、T2、T3时,试验组MAP、HR、TNF-α、IL-6水平和CERO2均显著低于参照组(P<0.05)。结论全麻及CPB下行心脏瓣膜置换术患者术中应用右美托咪定可有效减轻其围术期炎症反应及脑损伤,稳定血流动力学。 Objective To analyze the effects of dexmedetomidine on inflammatory mediators TNF-α,IL-6 and cerebral oxygen extraction rate(CERO2)during cardiac valve replacement under general anesthesia and cardiopulmonary bypass(CPB).Methods A total of 102 patients with cardiac valve replacement admitted in our hospital from December 2017 to December 2019 were included and randomly divided into experimental group and reference group,with 51 cases in each group.At 10 minutes before anesthesia induction,the reference group was given equal speed and equal volume of normal saline,and the experimental group was given 0.5μg/kg dexmedetomidine.Hemodynamic indexes,inflammatory reaction indexes and CERO2 were compared between the two groups.Results At T1,T2 and T3,MAP,HR,TNF-α,IL-6 levels and CERO2 in the experimental group were significantly lower than those in the reference group(P<0.05).Conclusion Dexmedetomidine during cardiac valve replacement under general anesthesia and CPB can effectively reduce perioperative inflammatory response and brain injury,and stabilize hemodynamics.
