摘要
目的 探讨右美托咪定对高龄髋关节置换患者围术期肝肾功能的影响.方法 纳入2019年5月~2020年5月进行髋关节置换术的患者60例,采用随机数字表法分为右美托咪定治疗组(A组)和对照组(B组),各30例.A组患者给予右美托咪定治疗,B组予等容量生理盐水.比较两组患者肝肾功能指标(AST、ALT、Cr、UREA、CysC)和糖代谢指标(Lac、Glu),判断右美托咪定对高龄髋关节置换患者围术期肝肾功能的影响.结果 B组患者术中和术后血清AST、ALT、Cr、UREA和CysC水平均显著上升,右美托咪定治疗可以显著降低患者AST、ALT、Cr、UREA和CysC水平,差异具有统计学意义(P<0.05);手术过程中,A、B两组患者Lac和Glu水平均无显著变化,右美托咪定治疗也无显著影响.结论 右美托咪定用于高龄髋关节置换术患者可以显著降低患者围术期的肝肾功能相关指标的表达,从而发挥肝肾功能的保护作用.
Objective To investigate the effect of dexmedetomidine on liver and kidney function in elderly patients with hip replacement.Methods A total of 60 patients who underwent hip replacement in our hospital from May 2019 to May 2020 were included,and 30 patients were divided into dexmedetomidine treatment group(A group)and control group(B group)by random number table method.Patients in group A were treated with dexmedetomidine,and patients in group B were given normal saline with equal volume.The liver and kidney function indexes(AST,ALT,Cr,UREA,CysC)and Glucose metabolism indexes(Lac,Glu)of the two groups of patients were compared to determine the effect of dexmedetomidine on liver and kidney function in elderly patients with hip replacement during perioperative period.Results The serum levels of AST,ALT,Cr,UREA and CysC in the B group were significantly increased during and after operation.Dexmedetomidine treatment can significantly reduce the levels of AST,ALT,Cr,UREA and CysC in patients,the differences were statistically significant(P<0.05);there was no significant change in the levels of Lac and Glu in the B group during the operation,and there was no significant effect of dexmedetomidine treatment.Conclusion Dexmedetomidine used in elderly hip replacement patients can significantly reduce the expression of liver and kidney function related indexes in patients during perioperative period,so as to play a protective role in liver and kidney function.
