小胰腺癌患者18氟-氟代脱氧葡萄糖正电子发射断层成像/计算机断层扫描影像学特点及其与临床分期的关系研究

摘要 目的探讨小胰腺癌患者18氟-氟代脱氧葡萄糖(18F-FDG)正电子发射断层成像/计算机断层扫描(PET/CT)影像学特点及其与临床特征的关系。方法将63例行18F-FDG PET/CT的胰腺癌患者根据肿瘤最大直径分为对照组(肿瘤最大直径﹥2 cm,42例)与观察组(肿瘤最大直径≤2 cm,21例)。比较两组影像学特点,分析不同分期患者的原发灶肿瘤代谢体积(MTV)、最大标准摄取值(SUVmax),并分析二者与临床特征的关系。结果观察组患者早期SUVmax、延迟SUVmax值均明显高于对照组,CT值、远处转移发生率及瘤体处胰腺形态异常比例、瘤体界限不清比例均明显低于对照组,差异均有统计学意义(P﹤0.01)。随着胰腺癌分期的升高,MTV值逐渐升高(P﹤0.05)。不同分期胰腺癌患者的SUVmax值比较,差异无统计学意义(P﹥0.05)。MTV值与胰腺癌分期、SUVmax值均呈正相关(r=0.396、0.450,P﹤0.01),SUVmax值与胰腺癌分期无相关性(r=0.195,P=0.08)。结论胰腺癌原发灶MTV与临床分期、SUVmax值有一定的相关性。胰腺癌患者早期行18F-FDG PET/CT检查测得肿瘤原发灶MTV在一定程度上有助于胰腺癌的临床分期诊断。 Objective To investigate the correlation between the 18F-fluorodeoxyglucose(18F-FDG)positron emission tomography/computed tomography(PET/CT)and clinical characteristics of patients with small pancreatic cancer.Method This study included 63 patients with pancreatic cancer examined with 18F-FDG PET/CT imaging who were then grouped by the maximum tumor diameter as control group(maximum tumor diameter>2 cm,n=42)and study group(maximum tumor diameter≤2 cm,n=21).The imaging characteristics of patients in the two groups were compared,to evaluate the metabolic tumor volume(MTV)and maximum standardized uptake value(SUVmax)in primary lesions,and to determine the relationship with clinical characteristics.Result The early SUVmax and delayed SUVmax in study group were significantly higher than those in the control group,while the CT value,the incidence of distant metastasis,the proportion of abnormal pancreatic morphology and unclear tumor boundary were significantly lower than those in the control group(P<0.01).With the increase of tumor stage,the MTV value of patients elevated significantly(P<0.05).The SUVmax value in pancreatic cancer patients at different stage were generally similar(P>0.05).MTV was correlated with pancreatic cancer stage and SUVmax(r=0.396,0.450,P<0.01),while SUVmax was not correlated with pancreatic cancer stage(r=0.195,P=0.08).Conclusion There is a certain correlation between MTV of primary pancreatic cancer and its stage and SUVmax.The MTV of primary tumor detected by 18F-FDG PET/CT examination is helpful for the clinical staging of pancreatic cancer.
