摘要
目的研究腹腔镜辅助下直肠外翻拖出式手术治疗直肠癌的临床效果。方法选取2018年2月至2019年2月郑州市第一人民医院收治的72例直肠癌患者为研究对象,根据随机数字表法分为两组,每组36例。对照组接受腹腔镜直肠前切除直肠癌根治术,研究组患者进行腹腔镜辅助下直肠外翻拖出式手术治疗。比较分析研究组与对照组患者的住院时间、肠功能恢复时间、手术时间、术中出血量及并发症发生情况等临床指标。结果研究组患者发生腹腔出血、肠梗阻及吻合口瘘等并发症发生率低于对照组,差异有统计学意义(P<0.05)。研究组与对照组患者住院时间、肠功能恢复时间、手术时间、术中出血量等临床指标比较差异有统计学意义(P<0.05)。研究组患者的局部复发率为2.78%(1/36),对照组为16.67%(6/36),差异有统计学意义(P<0.05)。结论应用腹腔镜辅助下直肠外翻拖出式手术治疗直肠癌的效果理想,安全性高,患者术后并发症发生率低,预后效果显著,具有临床推广的意义。
Objective To study the clinical effect of laparoscopic assisted eversion pull out operation on rectal cancer.Methods Seventy-two rectal cancer patients in Zhengzhou First People’s Hospital from February 2018 to February 2019 were randomly divided into two groups,with 36 cases in each group.The control group received laparoscopic resection of rectal cancer.The patients in the study group were treated with laparoscopic assisted eversion.The time of hospitalization,the time of intestinal function recovery,the time of operation,the amount of intraoperative bleeding and the incidence of complications were compared between the study group and the control group.Results The incidences of abdominal bleeding,intestinal obstruction and anastomotic leakage in the experimental group were significantly lower than those in the control group(P<0.05).There were significant differences between the study group and the control group in the time of hospitalization,the time of intestinal function recovery,the time of operation and the amount of bleeding during operation(P<0.05).The local recurrence rate was 2.78%(1/36)in the experimental group and 16.67%(6/36)in the control group,the difference was significant(P<0.05).Conclusions The clinical effect of laparoscopic assisted eversion and pull-out operation on colorectal cancer is ideal,with high safety,low incidence of postoperative complications and significant prognosis,which has clinical significance.
