摘要
目的探讨成人分泌性中耳炎治疗过程中常规药物联合物理治疗(包括捏鼻吞咽/按压耳屏)的疗效和价值.方法回顾性分析我科诊治的123例成人分泌性中耳炎患者的诊治过程,在常规药物治疗的基础上,根据是否遵医嘱联合物理治疗分为实验组(常规药物联合物理治疗)和对照组(单纯常规药物),比较两组患者的疗效、病程、并发症及后遗症、手术干预几率及复发率.结果联合物理治疗的实验组的总有效率98%,高于对照组93.15%;在平均病程方面,实验组(12.74±13.00)d,短于对照组(47.79±116.66)d,而实验组手术干预率(4%)低于对照组(16.44%),两组之间有统计学差异;复发率(6.00%vs 6.85%)方面在实验组和对照组之间无统计学差异,随访过程中没有发现明确并发症及后遗症.结论在成人分泌性中耳炎患者的日常治疗过程中,在常规药物治疗的基础上重视联合物理治疗(捏鼻吞咽/按压耳屏),可能有助于患者获得良好疗效,缩短患者的病程、加速康复,降低未来手术干预的可能性,是一种简单、有效、可行的联合治疗手段,值得在诊治过程中加强宣教和推广.
Objective To investigate the efficacy and value of the physical therapy together with routine drug therapy for otitis media with effusion(OME)in adults.Methods A retrospective analysis was made on the treatment of 123 cases of adult secretory otitis media patients in our department.On the basis of routine drug therapy,the patients were divided into the experimental group(routine drug therapy combined with physiotherapy)and the control group(simple routine drug therapy).We compared d iff ere nces in the curative effect,course of disease,complications and sequelae,surgery intervention and recurrenee rate between groups.Results The total effective rate(98%)of the combined physiotherapy treatment group was higher than that(93.15%)of the control group.The average course of disease(12.74±13.00)days was shorter than that(47.79±116.66)days of the control group,and the surgery intervention rate(4%)was lower than the control group(16.44%);there were statistically significant difference between groups.There were no statistical difference in recurrence rate(6.00%vs 6.85%)between the two groups and no complications and sequelae were observed during the follow up.Conclusion In the daily treatment process of the adult otitis media with effusion,paying more attention to the effect of routine drug therapy together with physiotherapy(pinched nose swallowing/tragus pressing),is conducive to help patients acquire good curative effect,shorten the course of disease,accelerate rehabilitation,reduce the possibility of future surgical intervention.This treatment is a simple,effective and feasible means of combination therapy,and it is worthwhile to strengthen the health education and promotion in the clinical practice.
