摘要
目的探讨尿毒症血液透析患者血清肿瘤坏死因子(tumor necrosis factor-α,TNF-α)、白介素-6(interleukin-6,IL-6)的变化及其与左心室肥厚的相关性。方法选择我院肾内科和血液透析中心收治尿毒症患者120例作为研究对象,随机分为两组,各60例。研究组采用高通量血液透析治疗,对照组采用低通量血液透析治疗。治疗6个月后,比较两组患者治疗前后血清TNF-α、IL-6水平及左心室质量指数(Left ventricular mass index,LVMI),并分析血清TNF-α、IL-6水平与LVMI的相关性。结果血液透析治疗后,两组患者血清TNF-α、IL-6水平均较治疗前下降(P<0.05),研究组下降更为显著(P<0.05);治疗后,对照组患者LVMI无下降(P>0.05),研究组LVMI则低于治疗前(P<0.05)。相关性分析显示患者血清TNF-α、IL-6水平与LVMI呈正相关(P<0.05)。结论尿毒症患者血清TNF-α、IL-6水平与左心室肥厚具有相关性。血液透析有助于降低尿毒症患者血清TNF-α、IL-6水平,高通量血液透析降低作用更为明显。高通量血液透析可能在一定程度上缓解尿毒症患者左心室肥厚。
Objective To investigate the changes of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and their correlation with left ventricular hypertrophy in uremic hemodialysis patients.Methods 120 uremic patients admitted to our hospital's nephrology department and hemodialysis center were selected as research objects and randomly divided into two groups,60 cases in each group.The study group was treated with high flux hemodialysis,while the control group was treated with low flux hemodialysis.After 6 months of treatment,the levels of serum TNF-α,IL-6 and Left ventricular mass index(LVMI)before and after treatment were compared between the two groups,and the correlation between serum TNF-α,IL-6 and LVMI was analyzed.Results After hemodialysis treatment,the levels of serum TNF-α and IL-6 in the two groups decreased compared with those before treatment(P<0.05),and the decrease in the study group was more significant(P<0.05).After treatment,LVMI in the control group was not significantly decreased(P>0.05),while LVMI in the study group was significantly lower than that before treatment(P<0.05).Correlation analysis showed that serum TNF-αand IL-6 levels were positively correlated with LVMI(P<0.05).Conclusion Serum TNF-αand IL-6 levels in uremic patients are correlated with left ventricular hypertrophy.Hemodialysis is helpful to reduce the levels of serum TNF-αand IL-6 in uremic patients,especially in high-flux hemodialysis.High-flux hemodialysis may alleviate left ventricular hypertrophy in uremic patients to some extent.
