摘要
目的探讨微卡联合2HRZE2HR超短程方案治疗初治涂阳肺结核(TB)的临床效果及对患者生活质量的影响。方法将66例初治涂阳TB患者根据随机数字表法分为对照组与研究组,各33例。对照组采用2HRZE4HR方案治疗,研究组采用微卡联合2HRZE2HR超短程方案治疗。比较两组的治疗效果。结果研究组肺部病灶吸收效果优于对照组(P<0.05)。研究组的痰涂转阴率高于对照组,随访1年后复阳率低于对照组(P<0.05)。治疗后,两组的FEV1%pre、FEV1/FVC、各项生活质量评分均升高,且研究组高于对照组(P<0.05)。结论微卡联合2HRZE2HR超短程方案治疗初治涂阳TB的临床效果确切,可有效促进肺部病灶吸收和痰涂转阴,改善患者的肺功能,提高其生活质量,预防痰涂复阳。
Objective To investigate the clinical effect of mycobaeterium vaccae vaccine combined with 2 HRZE2 HR ultra short course regimen in the treatment of newly-treated smear positive tuberculosis(TB)and its influence on the quality of life of patients.Methods A total of 66 cases of newly-treated smear positive TB were divided into control group and study group according to the random number table method,with 33 cases in each group.The control group was treated with 2 HRZE4 HR regimen,while the study group was treated with mycobaeterium vaccae vaccine combined with2 HRZE2 HR ultra short course regimen.The therapeutic effects of the two groups were compared.Results The absorption effect of lung lesions in the study group was better than that in the control group(P<0.05).The sputum smear negative conversion rate in the study group was higher than that in the control group,and the re-positive rate after 1-year of follow-up was lower than that in the control group(P<0.05).After treatment,the FEV1%pre,FEV1/FVC and the quality of life scores of the two groups increased,and those in the study group were higher than the control group(P<0.05).Conclusion Mycobaeterium vaccae vaccine combined with 2 HRZE2 HR ultra short course regimen in the treatment of newly-treated smear positive tuberculosis has definite clinical effect,it can effectively promote the absorption of lung lesions and sputum smear negative conversion,improve the lung function and the quality of life of patients,and prevent sputum smear re-positive.
